Evidence-based practice (EBP) and research are quintessential guides that professional nurses use in the quest to deliver the best care and achieve optimal outcomes. The aim of EBP is to aid practitioners to structure care of patients with the best and most current evidence. The first step and basis of EBP is to determine there is a gap in the knowledge base to be able to ask an answerable question by forming a focused clinical question. The most useful tools to assist in forming a focused clinical question is the PICO model. This tool can be used to construct searchable questions as the initial step in using research data to guide decision-making. The tool lists search terms to look at the connection around an issue and intercessions to address the issue. Pico is a critical mnemonic that is used to form the vital parts of a well-built, clinical question. In 2011, a study was performed to determine the level of nursing knowledge concerning EBP and if nurses were advocates for EBP. EBP is now accepted as the gold standard for professional nursing practice because patient outcome has improved (Flynn, 2015). A report from the Institute of Medicine (IOM) in 2003 states that EBP is considered a core competency and in 2010 confirms EBP is a basic competency. On a broad scope, keeping up with the latest evidence-based practice in healthcare can be a difficult task for nurses. The amount of evidence-based information available can be overwhelming. When you compound that with the challenge of finding the latest information on a smaller scope in a specialized area of the medical field it can be even more daunting. One tool that nurses can use to make that search easier is to formulate a clinical question in the PICOT format. A PICOT question is designed to make searching for that information easier by gathering feedback from others. This paper will define what a PICOT question is, identify a need for change in my current work environment and formulate a PICOT question to help me gather evidence to help facilitate that change.
An example of evidence based practice would be to find relevant studies for a specific topic area being researched. For example, to find the research paper titled ‘The efficiency of cotton cover gowns in reducing infection in nursing Neutropenic patients,’ the journal in which the article was published in this case, The International Journal of Nursing, would need to be found. Medline, an online database would be one method of finding this. A list of relevant search terms would then need to be researched by using the PICO Framework, this limits the search to only relevant items and ensures that a well built clinical question will then be formulated (Drummond,1998).
