Throughout these years, I have been faced many challenges. I started with a newly creative position right afterward graduated from college, decided to start a second career as a nurse, transition from a student nurse to a registered nurse, and lost my love one. All of these life events helped me to draw my career plan. After I had graduated from Stony Brook, I was hired as a care manager in an Asian community health center. This position was newly creative at that time, and this was also my first full-time job. As a Care Manager, I worked closely with patients, doctors, nurses, educators, administers, and other health care staff. Most of the Asian patients believed that the insulin is the last treatment for diabetes, and they are not able to maintain their current job performance when they have to start on insulin. Therefore, I discussed with nurses and educators to expand the education topics to clarify the misunderstanding of insulin usage in a community workshop. In addition to this, I found out some of the patients was uninsured or underinsured. They were experiencing financial difficulty to get their medicines and medical equipments especially if they were on multiple medications. Therefore, I discussed with my clinical director to expand their original medication assistant program. I reached local pharmacy and searched online drug stores to get a discounted price for a list of standard medication and medical equipments such as Glucometer and blood pressure monitor. I also promoted the medication assistant program at the community workshops and good health day. The program initially started with four to six patients. The number of the patient enrolled the program was fourfold six months after the program expanded. Our patients appreciated our hard work very
