Application of Middle Range Theories
Introduction
Nursing as a profession entails the utilization of knowledge and experience gained in the nursing field. This profession involves the use of concepts or ideas that are considered part of refined theories formulated a long time ago. The middle range theories are one of the critical pillars of these theories. The middle range theories are a group of nursing theories that are often limited in scope and their concepts, but more importantly, they provide a bridge between nursing theories and nursing practice (Kolcaba, 2001). These theories address not only specific situations, but also prove to be useful when addressing the challenges the most vulnerable segment of the population endure (Peterson and Bredow, 2010).
Theory creation is a powerful tool of the invention that involves conceptualization of ideas in a structured format with an aim of illustrating the conceived ideas in a sequential manner. This is possible with either the application of an

An in-depth description of the theory will be followed to establish and explain the concepts. Finally, an application of the theory concepts to a real-life nursing situation will follow afterward. The comfort theory is a middle range theory, and its concepts include patient healthcare, institutional integrity, institutional outcomes, intervening variables, health seeking behaviors, nursing interventions, and patient comfort. Before advancements in medical discovery in early 20th century, comfort theory was widely used in nursing environment. These comfort measures were administered by physicians, and had significant improvements in both the patients’ mental and physical condition (Peterson & Breadow, 2009). With the discovery of modern medicines and methods, comfort application reduced drastically. However, comfort theory is still applied to mentally ill patients. (Peterson & Breadow,
In order to understand middle range theories, a practical knowledge of the definition should be achieved before attempting to applying to concepts or classifications to the models. Middle range theories are defined as, theories that are fundamentally individual while incorporating a controlled amount of assumptions furthermore, having a limited characteristic of reality. These concepts are defined and may be tested (McEwen & Wills, 2011 p. 35).

Theory’s responsibility is to provide nurses with standards that reinforce practice, as well as, for future nursing understanding and delivery. Basically, it provides nursing professionals with a tested way of thought on how to handle certain situations with proven results. The importance of nursing theories to nursing research is the knowledge offered gives nurses the foundation for communicating with others and best practice. Middle range theory according to McEwen & Wills (2011, p 35) are theories that have concrete concepts, that are specific, incorporate a measured number of concepts and characteristics of the real world and are tested for accuracy.
& Willis, E. M. explains the middle-range theories were first introduced in sociology in 1960’s then were offered in nursing in 1974 focusing on emerging disciplines because they were available to introduce through research in compare to grand theories. Middle-range nursing theory was established fifteen years later, and is reinforced by repeated criticism of the summary of grand theories and their difficulties in use to research and practice. The reason of middle-range theory is to define, clarify, and predict occurrences that easer to apply in practical situations, and potentially to direct nursing interventions, change circumstances of situations to improve the care and potentially best outcome. Focus on middle-range theory is supported by theoretical framework for research studies, tested by research and use in practice, and scientific end product expressing nursing knowledge. The progress of nursing theories reflects the development of nursing science. Theories go beyond describing professional abilities, and aim for a synthesis which in turn becomes a reference to practitioners. This interplay between theory and practice currently mirrors specific features of our profession: its focus on the individual, the behavior, and the importance of the experiences, considered in a universal way. In consequence, the biological, psychosocial, cultural and spiritual connections of the human beings are the focus of the nursing discipline. This paper offers a nursing view, analyzing main concepts of the professional nursing roles.
Nursing is a complex profession full of challenges and rewards. To grow as professionals, it is important that nurses know and understand the many theory’s that exists in nursing and respect their impact on the profession. It is essential for nurses to learn from past theorist for nursing to move forward. This paper will correlate the philosophy and values of two nursing theorist with a unique perspective of each concept and define the nursing metaparadigm.
To make good nursing decisions, nurses require an internal roadmap with knowledge of nursing theories. Nursing theories, models, and frameworks play a significant role in nursing, and they are created to focus on meeting the client’s needs for nursing care. According to McEwen and Wills (2014), conceptual models and theories could create mechanisms, guide nurses to communicate better, and provide a “systematic means of collecting data to describe, explain, and predict” about nursing and its practice (p. 25). Most of the theories have some common concepts; others may differ from one theory to other. This paper will evaluate two nursing theorists’ main theories include Sister Callista Roy’s
Because these theory are known to have direct linkages to research and practice Most of these theory are developed from research, observation and experiences I do believe that Most nurses will agree with me if I say that the evidence based practice is a main engine for health well-being .Middle range theory signifies a growth of knowledge development in Nursing. Middle range theories offer valuable organizing frameworks for phenomena being researched by interdisciplinary terms. These theories are useful to nurses and persons from other discipline in framing phenomena of shared concern. (Archive, 2015).Middle-range theories are useful in addressing the problems of nursing, especially among vulnerable populations. Although middle-range theories address specific phenomena within nursing practice, the theories are broad enough to be applied to a variety of patient populations, and across many practice settings (Mareno, 2015). The language of middle –range theories is that used in nursing practice to deal with patient care phenomena such as pain ,unpleasant symptoms, empathy, uncertainty, comfort, change, lifestyle health promotion, relationships ,and deliberative planning for
Nursing behaviors that improve patient comfort are as simple as positioning and repositioning, knowing patients special comfort habits, and advocating for family presence (Kolcaba & DiMarco, 2005). It is important to use a measurement tool to assess comfort and remember to reassess for a positive outcome or plan a new intervention. Comfort is associated with the pursuit of healthy behavior, increased patient satisfaction, and better cost-benefit ratios (Kolcaba & DiMarco, 2005). Although Watson’s caring model is a broad philosophy that applies to nursing practice in general, nurses utilizing this theory would find Kolcaba’s middle-range comfort theory aligns well within the framework those

The best way to look at nursing theories is like the foundational block. Nursing theories are important set the tone of how a nurse will practice. A nurse will use intuition, practice, past expertise and events, and couple with learned theories to work every day in order to give the best patient care. it is all the more important to appreciate what first advanced nursing beyond mechanisms of practice to becoming a knowledge-based force in healthcare: That force is nursing theory and the theoretical thinking and research that generate theory. The complexity and depth of nursing are reflected in its structure of knowledge, which includes discipline-specific components such as philosophies, theories, and research and practice methodologies”( Reed, 2006). Patient care is a wide topic, but a key role in a patient’s care is the patient themselves, an educated patient is vital to their well being and higher level of care.

There are set definitions of nursing made by every nurses association and dictionary. However, nursing goes beyond the definition and has distinct meaning to each individual nurse whether they are aware of the meaning or not. Several nurses have chosen to publicize their beliefs about nursing into their own nursing theory. Other nurses may have ideas similar to these theories, completely different from the theories, or a combination of multiple theories. Some nurses may have never even considered what nursing truly means to them. However, it isn’t until nurses sit down and truly look at what nursing means to them that they can fulfill their duties as a nurse to their full potential. In any philosophy of nursing there are four metaparadigms
Healthcare professionals must remember that although their following a proven set of guidelines, it is important to treat each patient as an individual as well. The nursing theorists have taken individuality in care into account and mentioned the importance of structuring nursing based on each individual’s needs, (Wadensten and Carlsson, 2003). Diiferent theorists have come up with different points on view on the practice of nursing. For example, Martha Rogers and Betty Neuman are both theorists that developed different theories to describe human-environment interaction. Martha Rogers believed in using three principles; reasonancy, helicy, and integrality to predict human behavior influencing healing. Reasonancy, which relates to wave patterns; helicy, which is concerned with non-repeating rhythmicities; and integrality, which is the continuous mutual human field and environmental field process (Chapman, 1987). Rogers also believed that a patient environment has a direct effect on the healing process. Betty Neuman believed in a holistic view and that we must treat patients as a whole. Neuman also describe nursing interventions as three principles primary, secondary and tertiary preventions. Primary referring to the protection and strengthening of the line of defense, secondary prevention refers to increased resistance factors and reduction in reaction. Tertiary prevention refers to the patients
