On Wednesday, at approximately 0730 hours, I responded to 5018 Alamanda Drive, Melbourne, Brevard County, Florida 32940 in reference to a disturbance. Upon arrival, contact was made with Ms. Irene McDonald (Complainant), who advised she just had an altercation with her granddaughter, Ms. Rachel Anderson (Involved Person 1), and she was seeking advised on a drug treatment program. According to Ms. McDonald, she went into her granddaughter’s bedroom this morning to discuss the prescription medications her granddaughter was taking. As Ms. McDonald was in the bedroom, she listened to Ms. Anderson discuss her medications over the phone with her children’s father. Anderson, she advised the altercation started earlier when she was trying to get her children and her ready for school. According to Ms. Anderson, her grandmother (Ms. McDonald) came in her bedroom, locked the bedroom door, started yelling and pushing her. Ms. Anderson advised she had to defend herself by grabbing her grandmother’s hands and pushing her away to keep from being struck. Once she was able to escape the bedroom, she threaten to call law enforcement. At this time, Ms. McDonald called law enforcement for assistance. After Ms. McDonald called law enforcement, nothing else occurred between the two. Ms. Anderson advised she did not received any injuries and refused medical services. As I was taking photographs of Ms. Anderson, she again advised she did not receive any injuries. As I was taking photographs of Ms. McDonald, she advised she was open hand slapped in the left side of the face and her right arm was slammed against the wall, during the altercation. I took addition photographs of her left side of her face and right arm. When took photographs of Ms. Donald, I did not observe any visible injuries and she confirmed, once again, she did not receive any injuries. After completing photographs with both subjects, Ms. McDonald complained of swelling and bruising towards her left arm (outer left forearm area). When observed, it appeared to be a small area that was possible slightly swollen and discolored. Due to her age and skin condition, it was hard to observe. Addition photographs were taken of the area. I questioned Ms. McDonald once more if she needed medical services. She still refused medical services. I questioned Ms. Anderson about the possible swelling and bruising on McDonald’s left arm. Ms. Anderson advised she did not strike her grandmother. She believes she may have received the swelling and bruising during the time she was defending herself and trying to escape the
On the above listed date and time, deputies arrived at Hampton County Medical Center, and made contact with the complainant regarding the above listed incident type. Deputies gathered the pertinent information needed to complete this report. The complainant and his mother wrote a voluntary statement. Photos were taken while at the Emergency Room. The complainant was issued a Victim’s Right’s Form along with a case number attached. Deputies attempted to make contact with the subject, no contact was made. Deputies cleared the call and resumed normal patrol duties.

As the EAI team was discussing Molly’s case, one of the ED Residents made a few telephone calls. Molly’s PCP reported that during her last visit about 2 weeks ago, Mollie was alert and able to respond to questions appropriately. He confirmed that Mollie’s daughter and son in law have experienced psychiatric problems, adding that the son in law has expressed anger regarding Mollie’s living arrangements. The home health care agency was contacted. The RN and aide both report they have never met the son in law and have had very limited contact with Mollie’s daughter. When contacted by telephone, the daughter provided no explanation for Mollie’s extensive bruises noted on admission to the hospital. On the morning of the 17th of May 2005, Nola Walker was involved in a two vehicle motor accident. She had just dropped her son off at his new job, when she ignored a give way sign at an intersection. When the ambulance arrived the officers, Nucifora and Blake, recall Walker being “able to converse” and “orientated”. Blake conducted multiple assessments and did her vital signs twice. The results deemed Walker to be within normal ranges, with the only noticeable trauma involving superficial skin injuries on the left hand, an abrasion over the right clavicle which was assumed to be a seatbelt injury. DSS received a report of physical abuse, physical neglect and substantial risk of physical abuse on March 3, 2017 alleging there is chaos in the home. After returning with his brothers from their cousin’s house, the boys had to go to bed. The reporter stated that Xzavia banged on his mother’s bedroom door on three separate occasions wanting something to eat. The first time he was told to go back to bed and a popping noise was heard. The second time Ms. Kimberly Dawkins grabbed Xzavia by the head, shook him little and told him to go back to bed. The third time Ms. Dawkins grabbed Xzavia, threw him on the bed, got a gun, held him down and pointed it at him. Reporter states there was no clip in the gun. Ms. Dawkins was reported being upset and it was reported that she left the home for twenty minutes but came back. It was reported that while Ms. Dawkins was gone, Xzavia was very upset and he was crying and Tyzhaun stared punching holes in the wall. It was reported that Xzavia says he wanted to die and that Atmorris went and got a knife, stated to Xzavia, “You want to die?”, here’s the knife and the two older brothers, Tyzhaun and Atmorris left the house and Xzavia chased them. It was reported that Xzavia was outside crying and he spent the night with the neighbor.
Last week, Katera was taken into temporary custody and later released to Centerstone of Illinois. Kaella reported an argument ensued after Katera was denied permission to go to a friend’s residence located in Harrisburg over the weekend. Kaella moved from Harrisburg to her current address a few months ago. The argument started inside the residence. Kaella was in her bedroom and Katera was in her bedroom. Katera entered her mother’s bedroom screaming and yelling. Kaella could not control her daughter’s behavior. Katera hit her mother on the backside of the torso with a pot taken from the kitchen and threatened to kill her. Kaella remanded her young son to run into the kitchen and gather all the knives and kitchen utensils so that Katera could not get to them. At this point, Kaella called 911 for help. Kaella ordered Katera to go outside until the police arrived. Katera went outside and took out her anger on the vehicle. I did not observe any obvious damage to the vehicle that appeared
R/s Ja’Mya was telling other students her dad hit her in the head with a belt. R/s Ja’Mya stated that her dad was mad because she pushed her sister down, so her dad spanked her and hit her in the head with the belt. R/s she asked to nurse to look at the child’s head and the nurse said there is slight abuse near the hair line. R/s Ja’Mya told the nurse the bruise was from when she was spinning around and she hit the wall.

On Wednesday, December 28, 2016, approximately at 2:36PM, I responded to the ER, Room 15, in reference to an intoxicated patient that was being very disorderly with medical staff and officers. Upon arrival, I made contact with a patient that was identified as Burke W. Duncan, medical staff and officers. I observed officers standing in the hallway in front of this patient room. A nurse by the name of Allison Cockerham was caring for this patient and this subject became beligerent and started using all kinds of profanity and vulgar language towards her. This subject was so agigated that he started using profanity towards the doctors, nurse and officers on scene.
