The Colgan Air Flight 3407 was a very interesting case to look at. On February 12, 2009, at 10:17 pm, flight 3407 crashed at a house in New York after the pilots experience a stall. Flight 3407 was scheduled to fly from Newark, New Jersey to Buffalo, New York. The NTSB reported the cockpit voice recorder (CVR) revealed some discrepancies both pilots were experience. The first officer did not have any experience with icing condition but icing was one of the reasons the plane went into a stall. On the other hand, the captain had some experience flying in icing condition. The captain was experiencing fatigue, which indeed, made him unfit to recover from a stall. With that in mind, the Human Factor Analysis Classification System (HFACS) will give insight of some errors both pilots made.

There is camera proof of pictures of clouds and proof from the U.S. Coastguard of reasons for accidents in the area. The area is a dangerous part of the ocean for those reasons, but there is nothing mysterious about it. Also, there is no proof of the remains of people, planes, and ships because the Gulf Stream carries them elsewhere, but they have not disappeared, they have just been moved and altered. It is impossible for anything to stay in the exact spot it landed in the ocean because of the constant movement in the water. There is no evidence of the remains of people, but there are several contributing factors to why people have crashed such as: the underwater topography, the current of the Gulf Stream, and methane gas hydrates. There are very deep trenches in that area and it is unlikely that ships there will be found. The current in that area is also extremely strong and can misplace evidence of ships
In conclusion, many contribution factors led to the Crossair flight 3597 crash but is mainly triggered by Crossair’s incapability of assessment, pilot error and lastly the air traffic controller. Analysis of a flight crash is important so that we will know the causes, thus being able tackle it, making sure that there are no other flight crashes like Crossair flight 3597.
June 1st, 1999, captain Richard Buschmann and first officer Michael Origel were about to embark on their third and final leg of the day, after already working for ten hours on two other trip legs. They had arrived at Dallas/Fort Worth Texas around 20:10 CST and were eager to proceed on their final trip of the day to Little Rock, Arkansas. Poor weather in the region prevented their assigned aircraft from arriving on time, closely pushing them ever closer to their fourteen hour duty limits for the day. The first officer realizing their situation contacted the dispatchers to notify them they would need to find a substitute aircraft or the flight would need to be cancelled. Both pilots were well aware of the impending storms in the area, including around the destination airport, but they decided that they should be able to beat the storms there. This was one of the first signs of suffering from get-there-itis, along side of several hazardous attitudes. Once an aircraft was substituted, a McDonnell Douglas DC-9-82, the pilots were able to depart at 22:40, 2 hours and 12 minutes after their scheduled departure time, 12 hours into their 14 hour duty day.

It was the afternoon of July 25, 2000. One hundred passengers, most of them German, boarded the Concorde Air France Flight 4590. This was a trip of a lifetime for many people, as Concorde was restricted to the wealthy class of people. The excitement in people was cut short by the unfortunate delay in flight, because of maintenance in one of its engines. The passengers boarded the plane a couple of hours after the scheduled time. Finally, it was cleared for taxi on runway 26-Right. The pilots lined the aircraft parallel to the runway. A tragic accident, however, was about to befall.

At approximately 1:21 p.m., the communications and transponder signal were functioning no longer. There were no emergency calls at the time the plane had its last checkpoint. On March 24 at approximately 10p.m.Malaysia Standard Time, officials ceased the search as radio signals picked up debris found throughout a remote Indian Ocean. It is believed that all of the 239 passengers on the plane died (Preimesberger). From protests and outcries, to mourning and fallouts, it is still a distraught event that shocked the world.

It was unacceptable to many parties such as the stakeholders and the government especially the announcement was made at the same time as some of MAS regional competitors reported strong profits in the same year.

I thought it would be interesting to research information about why people go missing for myself and not just believe what is fed to me. I also find it very interesting how people can seemingly disappear out of thin air with no explanation. It is truly baffling how people can be gone forever with no one wondering where they went. I also do believe that the U.S. has a good process to finding missing people, and the only thing I would change is that officers should be sent out for every case, not just the most urgent ones. Families are still hurting and wondering where a member of their family went, even if they aren’t in high danger. If you are over 18 and do decide to go missing, just remember that you may not be breaking the law, but chances are you are breaking someone's heart.
