This Community report will focus on both a community assessment and a health promotion plan for Woy Woy Public school which is found in Woy Woy of the Central Coast, NSW. The report will target the children within the school as well as their parents and teachers. Through this assessment I will aim to build towards establishing a positive influence on the community’s subsystems through health promotion, determinants of health and primary health care. Within this community both children and their parents are closely related through both educational and social factors. ------ Health can be defined as “a state of complete physical, mental and social well being and not merely the absence of disease or infirmity. The enjoyment of the highest attainable standard of health is the one of the fundamental rights of every human being without discrimination of race, religion, political belief or economic and social condition” (WHO,2011). A community assessment is the first process in establishing health promotion programs and activities. This leads to assessment of a community’s health status, both positive and negative, which will assist in the development of health promotion (Francis et al 2007). It has been developed by the use of a social model of health, which considers causes of disease and lack of wellbeing on a holistic basis by addressing social, cultural, biological, environmental, political and economic determinates of health. A determinates approach incorporated understanding of how such behaviours affect both social processes and disease risks and how social and structural conditions enhance or diminish opportunities for communities and population to be healthy (Keleher & MacDougal 2011). After further research on CPNI’s it is clearly evident that they are important in both the acute hospital setting and community health care setting. The CPNI’s are an important aspect for those in the community but also for those in hospital preparing to go home. The CPNI’s were a wonderful and important aspect of the of the community assessment and health promotion plan of my community.
