On 08/05/2016 at approximately 1:50 AM, this Investigator with Investigator V. Shroyer arrived at 12211 N. Paradise Village Parkway, Phoenix, AZ for Case # 537001 report of Child Neglect for victim Fiona McFadden (DOB: 03-05-2013) against her mother Monica Katich (DOB: 09-17-89). Upon arriving on the scene, this Investigator met with the report source, Phoenix Police Officer E. Gomez # 7977 in reference to Phoenix Police DR: 2016-00001441605. According to Officer Gomez, Phoenix Police received an emergency 911 call from Monica Katich that her friend later identified as Ashley Brook Post (DOB: 06-21-83) had overdosed on Heroin. had overdosed on the Heroin that they had both been using. Monica stated that after Ashley began to have reactions from the overdose that she reacted by injecting Ashley with Methadone twice. During this time, it was observed by Officers Gomez, Cambell, and Prado (#7562) that a young child was moving around the apartment from her bedroom. Upon looking after the child, Officer Cambell saw syringes and a burnt spoon near a sink. Near the syringes was a silver lockbox. When Officer Cambell inquired about the box, Monica gave the officer permission to look inside along with the code of "2329". However, the box was already unlocked. The unlocked box contained Heroin, Methadone, and open syringes (see attachments). When Officer Cambell told Monica she was under arrest for an outstanding warrant, she revealed to Officer Cambell that she was also in possession of Heroin on her person. Monica Katich was arrested and booked into the Maricopa County Sheriff's Office 4th Avenue Jail for an outstand shoplifting warrant (Phoenix City Court), A.R.S.
13-3623 A2 "Child Abuse" a Class 3 Felony, A.R.S. 13-3408 A1 "Possession, use, administration, acquisition, sale, manufacture or transportation of narcotic drugs" a Class 4 Felony, and A.R.S. 13-3415 A "Possession, manufacture, delivery and advertisement of drug paraphernalia" a Class 6 Felony. While Monica was being arrested by Phoenix Police, Officer Prado watched Fiona McFadden. Officer Prado observed Fiona move a highchair towards a power box on the wall of the apartment near the apartment front door. Fiona then climbed and stood on top of her highchair and opened the lid to the power box and pointed to an extended black folding knife (see attached) with the blade out and stated: "this is daddy's knife." Officer Prado quickly removed the knife out of safety for Fiona as she was in reach of the extended knife. It should be noted that all of the narcotics in the apartment located by Phoenix Police were in reach and accessible to Fiona. Shroyer took Fiona McFadden into DCS state custody. A DCS Temporary Custody Notice and DCS Notice to Inform were served on Monica Katich and Christopher McFadden. This Investigator approached Officer Cambell who had taken custody of the folding knife located in the apartment and inquired about the length. Officer Cambell reported that the knife was 8 inches long and 4 inches in blade length. Officer Gomez approached this Investigator about having received the information of Monica's father and step-mother. This Investigator spoke with Monica in the back of the Phoenix Police vehicle to obtain her family's information. Monica inquired what was "going on" with Fiona. I informed Monica that the State of Arizona and DCS were taking custody of Fiona. Monica became upset and asked why she could not stay with her boyfriend Christopher McFadden (DOB: 11-24-87) who is the biological father to Fiona and also resides in the apartment. According to Monica, Christopher works at Well Fargo and would be home around 2:30 AM. Monica was advised that Christopher was not appropriate for placement, and the process that was already explained to her by Investigator V. Shroyer was again explained to her by this Investigator. Monica stated that she was in the prior care of Indiana CPS when she was a child and that there had been domestic violence from her biological mother. Monica denied any domestic violence between her and
Christopher. Monica also denied that Christopher was aware of her narcotic use. Monica provided this Investigator with her father's name "Daniel Katich" and step-mother's name "Lori Katich" who reside in Chicago. This Investigator and Investigator V. Shroyer took Fiona McFadden to DCS Placement Center and released physical custody of Fiona to them at approximately 3:10 AM. At approximately 4 AM, this Investigator was advised that Christopher McFadden had contacted the DCS Hotline requesting information. This Investigator contacted Christopher at (312) 399-7426 where it sounded as if a child was crying in the background. Christopher stated that there was no child and that he was driving. Christopher then asked what was going on and "how to get his kid back." Christopher stated that he did read the DCS TCN and Notice to Inform and that Monica was arrested for drugs. Christopher stated that he did not understand why he could not have Fiona since he was not arrested for Heroin. Christopher further stated that he gets home at 3 AM and in the past there had been no issues with narcotics or "overdoses." When this Investigator asked about Monica's drug use, Christopher stated that Monica has been to rehab in the past. When this Investigator inquired as to the times she was in rehab, Christopher became defensive and asked about getting an attorney. When this Investigator inquired as to Monica's Heroin use, Christopher became uncooperative and stated that he needed to charge his phone and for this Investigator to call him back in 15 minutes. Upon attempting to call Christopher back, his phone went straight to voicemail.
Anna Garcia is a thirty eight year old Hispanic women. She weighed 165 pounds and was 64 inches tall. She was married to Alex Garcia, but went through a nasty divorce. Anna has no children and one dog. A phone call came in at 9:45 AM on the hot morning of August 14th. Anna’s neighbor, Doug Greene let the police know he saw her walking her dog around 6:30AM yesterday morning, but heard her dog barking for the last two hours. Renee Heikamp, 19, and case worker from the Catholic Children’s Aid Society (CCAS), Angie Martin, were charged with criminal negligence resulting in the 1997 death of newborn baby, Jordan Heikamp. The charges were dropped shortly after Jordan’s death, due to a lack of evidence from the investigation of a 63-day inquest. (CBC, 2001). Renee Heikamp and her baby were residing at the Anduhyaun shelter that services Aboriginal women fleeing abuse during the time of his death. Jordan Heikamp had starved to death, weighing only 4 pounds, 4 ounces less than what he weighed at his pre-mature birth, in May 1997; a photograph shown to witnesses at the inquest revealed the corpse of the baby who was little more than a skeleton.
At admission, Mollie’s main complaint was right hip pain. She was not oriented to person, place or time, responding with “I don’t know” to questions asked. While the emergency department nurse completed a physical assessment, Molly’s hospital record was retrieved. Molly was discharged from the hospital two weeks ago, having been admitted for dehydration. Her health history was significant for hypertension and diabetes. Carla Washburn, an incredible, inspirational woman that has sadly witnessed the 3 closest male figures in her life all pass on. This has resulted in Carla becoming depressed over the unbearable experience. Carla embraced in giving back to the community and that’s not surprising because she’s a person of spirituality. While, Carla is the client and is suffering through grief, it’s likely she’d benefit immensely from creating a program to work with the kids in the community and may give her a sense of worth she’s been seeking since these 3 tragedies.
Ashley Smith was a young girl that was placed in a juvenile detention centre at age 15 for throwing apples at a mail man. Her short sentence quickly extended into a life sentence because of so many infractions within the prison system. Ashley suffered from extreme mental health issues and was place in a psychiatric prison facility, however this facility was shown in the documentary to be corrupt and their actions with Ashley were extremely illegal. Furthermore, Ashley wasn’t given the proper help and treatment that she needed, instead she was physically and verbally abused by guards in the prison, and she ultimately passed away in the prison. Post Miranda Scruggs stated that he was walking down Vinson Rd and saw a black car sitting in the front yard of a house. Scruggs stated he used a pair of pliers to enter the rear door of the house to try and find the keys to the car. Scruggs entered the house and found the keys by the refrigerator along with a purse that contained a debit card. Scruggs stated that he grabbed the items, and then found a small revolver in the kitchen drawer along with bottle of Methadone. Scruggs stated that he also got a flat screen tv. It was a hot 92 F summer morning. Doug Green, Anna Garcia’s neighbor/ex-boyfriend contacted the police, because he was worried about his next door neighbor, Anna Garcia. He had spoken to Anna the previous morning when he saw her walking her dog at 6:30 AM. It is usually a quiet suburban neighborhood. The emergency call came in at 9:45 AM. Anna Garcia was found dead in her home with her dog in her home at 9:56 AM. Anna was wearing a sweater during the heatwave. The reporting party (RP) reported foster mother Lori Steele located at 1048 Newhill Street, Glendora CA 91740 observed foster child Chateau Lane-Jones had been sending nude photos and video to men on "Facebook" and "Snapchat. Some of the replies from the mew indicated they were going to see Chateau. The replies included pictures of men. On 10/20/16 Chateau provided the name of her school and the foster parent's address. This information was provided by Chateau's foster sisters. The RP was not aware if the pictures the men submitted were nude or not. Subsequently, Jessie has access to the bio daughter's drug paraphernalia (heroin pipes) which are left in the bedroom. Per Reporter: Deroen was taking over his father's (F.M.) home for the Christmas Holiday on 12/21/2017. F.M. was arrested for molestation Deroen's nine years old sister (Shakira) in the past.
