Summary On November 27, 2017, after an extended medical leave, Mr. Keifer Bradshaw notified me in person that he had been officially released from the doctor and could return to class. Based on Mr. Bradshaw’s extended absence (last date of attendance 9/21/2017), missed assignments (approximately 25 assignments not including TA Presentations), and overall grade average prior to leaving class (22%), I recommended that Mr. Bradshaw seek a medical withdrawal from class. After consulting with the Registrar and his academic advisor, Mr. Bradshaw notified me in writing that he desired to be giving an Incomplete (I) for the course. I previously advised Mr. Bradshaw that this was against policy as Incompletes are only given when students have a passing
grade prior to their absence from class. Mr. Bradshaw’s overall grade average prior to his extended absence was 22% out of 100% (F). Due to the extreme circumstances regarding Mr. Bradshaw’s absence, I have agreed to issue an Incomplete (I) for MATH101-007 Fall 2017, although I feel other options, such as a Medical Withdrawal, would better position him to be successful in Math101-007. Based on his request, the attached documents include the list of assignments that need to be completed for the Incomplete to be removed. The Incomplete must be removed within six weeks after the beginning of the Spring 2018 semester.
Arizona employers who do not currently offer paid sick time will be required to start accruing as of July 1, 2017. There has been an enormous amount of debate over whether there is a need to offer sick time, especially to employees who have paid time off which can be used for anything, including sick days. On the other hand, there is a significant need for employees who are not offered any paid sick time or paid time off. You will find a mix of employees and employers on either side and sometimes on both sides of this debate. Few will change their opinion when they think of themselves as the employee versus the employer and vice versa. Employers and employees have raised concerns alike regarding whether providing days specifically denoted as sick will encourage more unplanned days off, will negatively impact those that do not get sick, and whether it is fair to require sick time versus paid time off in which we can all agree everyone should have paid sick time when the
Over the last several years, the United States has experienced the emergence of countless lifestyle based illnesses. We have seen an increase of inactivity, poor nutrition, an increase of tobacco use, and more frequent alcohol consumption; Americans are living unhealthy lifestyles that are surely factors contributing to the pervasiveness of chronic diseases. Chronic diseases use to be seen as problems that older generations faced, but over the past several years the United States’ working age group has become a main target. For example, Americans are experiencing higher rates of diabetes, and heart disease. Conditions like these only lead to a diminished quality of life, could possibly lead to shorter life spans, disability, and quite possibly in the long run increased health care costs. Businesses are also impacted by an employees lifestyle decisions; absenteeism and presenteeism.
The case of sick leave involved Kelly, who was hired by Soto Board of Education in Japan. Her employment was with The Japan Exchange and Teaching Program (JET) and Conference of Local Authorities for International Relations (CLAIR) had taken over the contracts for the workers of JET. CLAIR’s role was to be a liaison during recruiting and selection, counseling and participant welfare. Their main function was to promote the exchange at the local level. The paper stated that if there was a problem with the host institution, then it was up to the host to address and solve the issue. If the problem didn’t get resolved then the participant could go to CLAIR with their grievances. It was up to them to step in on JET participants’ behalf to work to solve the issue.

According to the Centers for Disease Control and Prevention (CDC) (2012), the average time patients spend in the U.S. emergency department (ER) before they can see a doctor has increased to 25% between 2003-2009. The main cause of longer wait times (WT) in the ER is overcrowding. Overcrowding has been found to be closely related to both subjective and objective patient satisfaction (Miro’ et al, 2003). Longer wait times in the ED is such an important issue because its consequences are detrimental not only to the ER patients, but also to providers. As a health care provider, decreasing patients’ WT in the ED is essential, although challenging, to improve patient’s health outcomes and increase patients’ satisfaction. Although it is a very challenging issue to tackle, hospitals that have initiated some quality improvement (QI) strategies are experiencing some positive outcomes in that area of care. The outcomes are measured by decreased waiting times, improved patients’ clinical outcomes and increased patients’ satisfaction.
Mr A could not bring himself to the hospital due to the extreme pain. He called an ambulance and he started to deteriorate en route to the hospital. His GCS score dropped, he started to get confused and then he threw fits.
The subject came so naturally to me and I decided to continue my enrollment in sports medicine courses throughout my high school education. As my senior year of high school concluded, I explored the idea of a career in physical therapy by job shadowing at the Idaho Sports Medicine Institute (ISMI). After observing the PTs at ISMI, I was immediately drawn to a career in physical medicine. A few months later, I started my education at Idaho State and felt exactly like I did when I started sports medicine in high school. I was absolutely enthralled with my classes, naturally excelled in all of my coursework, and always had PT school in the back of my mind. I knew I would have to work extremely hard to develop a strong undergraduate transcript and build a competitive application. Throughout my undergraduate career, I have worked relentlessly to maintain academic excellence and currently have a 3.91 GPA. I have also succeeded in making the College of Education’s Dean’s List for five semesters. There are many other things for Mr. Mocel to consider when making his decision about Lauren’s online test failure and his decision will ultimately reflect his morals. This ethical dilemma deals with professionalism, syllabus policies, and personal relationships. The stakeholders in this ethical dilemma would include Lauren, Mr. Mocel, other current students of Mr. Mocel’s biomechanics online course, other professors and administrators in the department of Exercise Science. In this specific ethical dilemma, the best solution for Mr. Mocel and Lauren is not so visible. There are many different people and situations to consider when making this decision for Lauren. The 3 following solutions address the dilemma that Mr. Mocel faces. These include, Mr. Mocel letting Lauren re take the final examination in hopes that Lauren keeps the re-take private, Mr. Mocel not letting Lauren retake the final examination because he doesn’t want to go against his policy but allowing her to complete an alternate assignment to achieve a passable, or that Mr.
I knew I needed to get back in school to make any advancements as well as earn more money at retirement, but wasn’t sure what direction I wanted to go in; should I continue in the education field or athletic training. While walking around at the National Athletic Trainers’ Association annual convention in the exhibitors’ expo, I discovered the Indiana Wesleyan booth. The athletic training program seemed interesting and fit into my hectic schedule with it being only one year. Therefore, as soon as I got home, I immediately checked the Georgia Professional Standards Commission website to see if IWU was on the approved programs for a teaching certificate upgrade. Unfortunately, it was not on the approved list, however a proposal could be sent to the commission to review the program. It wasn’t economically feasible for me to spend the money to receive a master’s degree with no return on my investment if the program wasn’t added. For almost a year, I checked the Professional Standards Commission website in hopes the program’s approval. You cannot imagine the excitement I experienced when I checked the website and saw Indiana Wesleyan University’s athletic training degree listed. I am respectfully writing a request of waiver of Rule G(1) of the student handbook, requiring students to maintain a 2.0 GPA to remain enrolled. During the Fall 2016 semester I failed one course, causing me not to earn the 2.0 GPA required to remain a student at Florida Coastal School of Law. Though I had a bumpy start at Florida Coastal, not doing as well as I hoped my first semester, I was proud of my improvement of my GPA during the summer. Fall for me was not as successful of a semester, I had several personal issues that affected my performance, including the death of my maternal grandmother and unforeseen health issues. Throughout the year, a high school teacher will remind the student of upcoming due dates and assignment requirements. If a student misses a test or homework assignment, the teacher will provide missing notes or study guides so that the student is kept up to date with the rest of the class. For example; when I was in high school, I missed a week of school to go on vacation with my family. My mother spoke with my teacher before we left and he agreed to let me turn in my homework when I returned to class. Furthermore, my teacher allowed me to make up the single quiz that I missed during my absence. Neither the late homework, nor the missed quiz reflected in my grade. Hope all is well! I have been getting emails about Fall 2015 registration and the need for me to register for Maintenance of Matriculation or Leave of Absence. I did not know that I needed to register even though I only have one HPM course left. I have been in touch with Cindy and Pamela regarding the problem and Pamela suggested a leave of absence if I decided not to register for Maintenance of Matriculation. I also read that student must obtain permission from the department chair prior to taking a leave of absence. Since the last day for registration is August 21st, 2015 ( I think), I will be on-campus tomorrow August 21st, 2015 to fill out the leave of absence form.
