SYNOPSIS: At hours on June 7, 2016, I, Cpl. Lessane along with Deputy Ayer, with the Hampton County Sheriff’s Office, responded to Hampton Emergency Room, in the Hampton area of Hampton County, in regards to an assault that occurred on Bryan Road, in the Hampton area of Hampton County. Upon arrival, Hampton County Sheriff’s Deputies made contact with the complainant, Ta’shanae Smith, who stated she was assaulted by her boyfriend, Antquon Robins. Ms. Smith was in Mr. Robin’s vehicle, a black Crown Victoria when a verbal dispute occurred when she received a telephone call from a male friend. She advised the dispute led to her ear being slice with an unknown blunt object. Ms. Smith was being treated by the local emergency staff. INTERVIEW WITH COMPLAINANT: (TA’SHANAE SMITH) Ms. Smith stated on the above listed date and time, she asked her boyfriend, Antquon Robins, to give her a ride to her mother’s on Bryan Road. Smith. Sheriff Deputies gathered the above listed statement in regards to the above listed incident type. Sheriff Deputies gathered the pertinent information needed to complete this report. Sheriff Deputies observed Ms. Smith left ear bleeding at the time of our presence. It appeared Ms. Smith ear was cut with an unknown blunt object. Photos were taken while on scene. Ms. Smith wrote a voluntary statement in regards to this incident. She was issued a Victim Right’s Form along with a case number attached. She was advised this incident would be documented on file at the Hampton County Sheriff’s Office. She also was advised a warrant would be attempted from the Magistrate’s. Deputies cleared the call for service and resumed normal patrol
On March 24, 2016, officers were dispatched to a scene where a male subject was trying to gain entry into a vehicle using a hammer. Upon arrival officers made contact with a male subject who was later identified as Keith Hunt, the defendant, and the victim. The victim explained to the officers she was standing near the trunk of her vehicle when Mr. Hunt approached, He attempted to keep into her vehicle without permission; so she confronted the defendant and tried to secure her vehicle. Mr. Hunt demanded she give him the keys and her wallet. The victim stated the defendant had a hammer in his hand and was threatening her with it while he was telling her to hand over the property. They began to struggle over the keys and the victim screamed
On the above listed date and time, deputies arrived at Hampton County Medical Center, and made contact with the complainant regarding the above listed incident type. Deputies gathered the pertinent information needed to complete this report. The complainant and his mother wrote a voluntary statement. Photos were taken while at the Emergency Room. The complainant was issued a Victim’s Right’s Form along with a case number attached. Deputies attempted to make contact with the subject, no contact was made. Deputies cleared the call and resumed normal patrol duties.

On September 18, 2015, I called Ms. Ashaunta Lanier and identified myself and the purpose of my call. Ms. Lanier informed me that she was about to call the police to report Tavon’s return. Ms. Lanier reported that her son had been in The District of Columbia where he was caught committing a robbery. Ms. Lanier reported that a search warrant had been executed and evidence which implicates her son had been recovered. Ms. Lanier authorized me to meet with Tavon at school.
Victim Avina told me approximately five years, prior to getting married, Suspect Avina threw his dress shoe at her. The shoe hit Vicitm Avina is the stomach which did not cause any injuries. About two months ago, Suspect Avina attempted to push Victim Avina’s right shoulder. Victim Avina lend back and avoided the push but hit her right upper thigh against the living rooms coffee table. The impact later left a quarter size bruise. Victim Avina took a photograph of the bruise (see attached photo).
On October 31, 2016 the complainant walked into the Fifth District police station and reported that on October 28, 2016, Tavante Robinson hereinafter referred to as the respondent had sent threatening texts to another teacher at the school. One of the text messages read “Don’t think it’s over u wanna be in this to say nm idgaf I got you Ms. tabias Ms. Hayward all yall mfs don’t think its sweet I left my mother ima be waiting for her to step foot out that mfn building ima get yall one by one.” The message was then followed by the defendant sending pictures of several guns and in one of the pictures he was holding a gun with an extended magazine. The respondent then sent another text messages stating “Tell Ms. Ashford to look into finding some new teachers staff period.” The complainant is afraid for her safety and believes that the respondent can carry out the threat. The respondent has mental issues and believes he has access to weapons. The complainant was provided these text messages a day later after the incident occurred.
Upon arrival I conducted a field interview with Williams who stated he and his girlfriend (Washington) were arguing because she was talking to him in a nasty manner, next the complainant stated Washington's 14 year old son (Aushawn Washington) started a verbal altercation with him. Next Williams stated Aushawn started shooting a BB gun at him so he started running after the child, afterward Williams stated he walked to the hood of his White Ford Expedition and Washington punched him in the mouth and that's when the two parties started fighting. Williams further stated Washington then attempted to get in the vehicle and when he tried closing the door Williams pushed it open striking her niece.
Lisa stated this morning at the listed location; she set up a yard sale table, along with several other neighbors. During the set-up process, the accused, Marion Wagner, approached her advising her she was not able to set up the table. Lisa advised she was able to be here since it’s on the sidewalk and that she wouldn't be any more than a few hours here. Lisa stated that Marion approached her, got up in her face, and then struck her in with a closed fist. Lisa then fell to the ground and started to bleed from her nose. Lisa is herein referred to as the victim. It's to be noted that Lisa refused any medical attention.
Ms. Peterson as a prior indicated report for cut/welts and bruises to Anthony on 5/6/1999. The summary indicates Anthony had bruising on his buttocks.
A. Working in the emergency room I encounter many patients who come in for psychiatric evaluation, wither it is due to suicidal thoughts or depression. Through the constant training that our hospital puts on, I have learned how to better deal with these types of patients who can be difficult at times.
When I arrived SB was in bed asleep. I woke SB up to give SB her medicine, SB received her 7 AM, 9 AM medication and eye drops. Around 10:10 Sheila informed me she was dizzy and needed to use the rest room. SB attempted to get up and to use her walker. SB made it to the hallway and was not able to walk anymore because her walker's back wheel was not working. I assisted SB to the bathroom. When leaving I had to catch SB a few times. I called the emergency line because Sheila was saying she was dizzy and her walker was not properly working. At 11:52 AM I was assisting SB out from the bathroom and I went back into the bathroom to clean up after her. While I was in the bathroom, I asked SB a few times if she was fine and needed any help. My overall learning experience during my preceptor shifts was amazing. The first day I walked into the Emergency Department for my shift, I was having anxiety through the roof and very nervous. I felt like I did not know anything and it was a completely new environment then I am use to. At this point I feel very comfortable in the environment and felt like I have gained the knowledge to be a competent nurse in practice. I owe a lot of the success I have had in the ED to my preceptor Sam. He was seriously great and very patient with me when I was trying to learn something.
