I had my fourth day of school nursing and it was very productive. I love my school nurse Brenda; she is so helpful. I love the tricks that she has taught me for assessments, such as if a child’s pulse is up, chances are something is going on. I also helped Brenda with completing the rest of the vision and hearing rechecks. And worked on interviewing my student for the IHP. Because I knew that my student’s strengths were creativity and communication, I decided to discuss why exercising is important for a diabetic patient. J.J. and I had a great discussion about how diabetics are more likely to acquire complication like high blood pressure, stroke, and cardiovascular disease. I found out that she was worried about becoming hypoglycemic because
last year, her doctor added metformin and along with her Humalog and became very sick. She is receiving Humalog and metformin together because her nurse practitioner thinks that J.J. liver may be over producing glucose, and the Humalog alone isn’t helping. J.J. and her mother state that this combination of medications has been working for her and she no longer is having uncontrolled hyperglycemia. However, she can get low easily, therefore, she a fear of becoming hypoglycemic when exercising. We discussed why it’s important for her to exercise and brainstormed ways, she would like to exercise. Because I knew that J.J. was creative, we brainstormed together about ways we could get her active; this involved walking with her dogs, riding her bike, walking/talking with her friend, and longboarding outside. Today, I learned, that she does play softball in the spring, so I was happy to hear that she is outdoors. Then we discussed ways for preventing her from becoming hypoglycemic; some interventions included checking her blood sugar before and after she exercises and possibly having a snack if she was low. I also advised her to build up her tolerance for exercise slowly. I felt good about our conversation. I truly want J.J. to be as healthy as she possibly can; she is a wonderful, bright, 12-year-old. I felt our discussion went well and I feel good, knowing that I am helping to make a difference in this little girl’s life.
The case study chosen for this assignment is case study #2: Hannah is a 10-year-old girl who has recently been diagnosed with Type 1 Diabetes Mellitus. She is a 4th grade student at Hendricks Elementary School. Prior to her diagnosis, Hannah was very involved in sports and played on the girls’ volleyball team. Her mother is concerned about how the diagnosis will affect Hannah.
I was then introduced to a patient who was in isolation. Her legs were immovable and were crossed in a very uncomfortable position. I wish I could’ve done something so that her legs could be in a more comfortable position, but all I could do was observe and get her a cup of ice cold water to drink. During this clinical observation, I didn’t get to see much but overall, it was a good experience. It made me realize what it was like to be in a hospital setting and what it meant to be a nurse. Seeing how the patients were still able to smile through all the pain they went through, it made me want to become a nurse even more because I would also like to make my patients happy. If I could do one thing differently during this clinical observation, I wish I didn’t ask my senior nurse about what externships she took and instead, I wished I asked her more questions about the patients in order to gain more information about
Diabetes mellitus also known simply as diabetes refers to a group of metabolic diseases which affect the body’s homeostatic mechanism used for maintaining and regulating the body’s blood sugar levels. Diabetes is a chronic condition which, in 2013 was estimated to be affecting 382 million people worldwide.[1] People suffering from diabetes are required to constantly be vigilant of their blood sugar levels to ensure it does not go below or above optimum levels. Depending on the type of diabetes and glucose level range, they may need to inject themselves with insulin or eat high sugar foods to restabilise their body. Failure to do so can lead to other long term health effects.[2]

Obesity, on the other hand, is a health condition whereby excess body fat has accumulated to an extent where it has a negative effect on health. This leads to reduced life expectancy and increased health problems. Diabetes and obesity are closely tied as doctors highly link diabetes to people’s weight problem. This study focuses on the relationship between obesity and diabetes and the care for patients suffering from these conditions.

Dear Mayor Blasio, As society develops, people do not stop seeking access to healthcare services in order to ensure a better quality of life. With largely diverse populations, the Bronx has emerged to be a widely populated community in New York City. As a result, the challenges present in the community are overpopulation, polluted environment, excessive fast food chains, and noise pollution due to various transportations that are a root cause of many healthcare problems.
The health problem I have chosen is Type 2 Diabetes. Research has shown that early detection of symptoms of diabetes is crucial and important as it may prevent further difficulties. It is difficult to always identify if symptoms are linked to Type 2 Diabetes because they are often less taxing on an individual’s day-to-day activities. Symptoms of Type 2 Diabetes include but are not limited to frequent urination, extreme fatigue, blurred vision, numbness in hands or feet, and feeling either very thirsty or hungry (American Diabetes Association, 2014).Risk factors of this health issue include but again, are not limited to, weight (being overweight is a high risk factor), lack of movement during daily activities, family history and genetics, race, and age (Risk Factors of Diabetes, Mayo Clinic, 2014).
What is diabetes? Should I be worried about getting it? What food can diabetics eat? Can they live as long as non-diabetics? Those were some of the questions that swarmed in my head when two of my close relatives were diagnosed with diabetes. After my relatives had been diagnosed, my whole extended family changed in how we ate and looked at food. I became very aware of this disease and thought I knew a little bit of what it was, but still had a lot of questions. I now feel like I have the knowledge of exactly what diabetes is, how to deal with it, and most important, preventing it.
It was a very slow environment and because of this I was a bit bored most of the day. The nurse spent most of the morning catching up on documenting incidences that happened the day before. Most of the activity that we saw when I was there happened right after lunch and everybody came in from the playground. The most enjoyable times for me was when we went to the special needs center to check on one of the kids with diabetes. The teachers in that classroom are amazing for being able to know how to teach and handle kids with learning disabilities. I learned that the school nurse is a lot slower environment and has its advantages if you are a parent. It is very important though, a lot of the kids that came into the office where just seeking attention and approval. A school nurse needs to be able to pick up when something is wrong with one of the students. My biggest weakness during the clinical experience was being able to communicate well with the kids. Since communication varies with the different age groups and there are so many different ages in the middle school, I had difficulty transitioning from talking to the older kids to the younger kids and vice versa. Since each of the kids grow at different rates it was also hard sometimes to tell which kids were the older ones. Throughout my final ten weeks at my placement, I have grown and overcome so many obstacles. I have accomplished a wide range of skills since the beginning and have been improving on them as I gained experience. At my placement as a student nurse, I have gained a lot of confidence, skills, knowledge and experiences that have helped me act and work in a professional way. All the experiences I have had during the ten weeks of my student years have helped me in shaping me into a professional.
One might wonder what exactly diabetes is. Most know that a person with the disease has to keep a close eye on his or her blood sugar. But, people who do not have diabetes do not know what it really is. Diabetes is the result of high blood sugar. Insulin helps the body get energy by absorbing carbohydrates in the foods people eat every day. Sometimes the body cannot use insulin efficiently or it just cannot make enough of it. This is when diabetes is developed. If glucose cannot be absorbed then it will build up in the blood. High blood sugar can damage nerves and blood vessels. It can lead to heart disease, stroke, kidney disease, blindness, dental disease, and amputations. One with diabetes can also be more apt to get other diseases, depression, and have issues with pregnancy. (NDIC, 2011)

Diabetes is a disease caused by a number of factors such as due to complete or partial loss of insulin production or complete or partial loss of its action or both. Whatever the type of diabetes, the main feature of all types of diabetes is elevated level of blood glucose termed as hyperglycemia. A disease forming alterations in capillary’s basement membrane thickening, elevation of matrix of vessel wall and proliferation cell leading to the vascular complications such as narrowing of the lumen, and early atherosclerosis. Diabetes is found to be related with the elevated generation of atherosclerosis that affects the arteries involve in the circulation to the heart, brain and lower extremities. As well as diabetic myocardial disease, is one of the complications of diabetes. Diabetes founds to affect about 100 million people all over the world. The conditions of diabetes leading to the introduction and generation of microvascular disorder that is results by the destruction of the small blood vessels and macrovascular disorders that results by the destruction of arteries. In diabetic patient the major reason of inability and death is the vascular disease mainly atherosclerosis.
In our society today, the number of people that are diagnosed with diabetes is increasing. Diabetes is caused by many factors like genetics, inactivity, and obesity. With obesity on the rise in our country, so is diabetes especially in young children. With the fattening and unhealthy foods provided by fast food chains and grocery stores, children are the ones that diabetes is affecting the most, by causing them to have higher chances of being diagnosed with diabetes. According to Kim and Lee (2008), the rates of childhood obesity have tripled over the past three decades. Childhood obesity is becoming a problem for many youth in our society, with more youth being overweight and inactive increases the chances for them to be diagnosed with diabetes. The community needs to become active about this topic by having more activities open to the youth in the areas. This would include workshops and sport programs for the youth being available in the community. Hopefully, the North Carolina School Diabetes Act will educate children so that they will learn to make good quality health decision when they are younger and to give them a better chance of not being diagnosed with diabetes.
