Which criteria are most important for use in determining community health improvement priorities and why? There are many important factors to take into account for use in determining which community health concerns should be a priority. Initially, a list of health concerns should be identified with the list narrowed down after the task force has determined what percent of the community population is at risk for the health problem, and what percent of the population has the health problem. Determining priorities means taking into account the trends and patterns in health data and to apply lessons learned from research. It is also crucial to include expectations from community residents, community partners and decision makers. The availability of resources, both monetarily and in manpower, to implement a possible priority must also be taken into consideration. The severity of the problem and number of people affected, disparity/equity and values are also a determining factor for establishing health priorities (APEXPH, 1991). Also, health problems that may cause death or have significant comorbidities are considered for determining community improvement priorities as is the “consequences of inaction “ (archived.naccho.org, n.d.). These criteria play an important role for establishing the top health priorities of a community. …show more content…
Each method has strengths and weakness and depends upon a strong facilitator to move the process a long. Community members should be included in any method that is establishing criteria ((APEXPH,
I believe that every citizen deserves good healthcare services regardless of his or her geographical area, income, or race. An underserved community is a community in which people are unable to obtain health care or have limited access to the health care system for different of reasons. These reasons include ethnic background, socioeconomic variables, lower salary in some areas, extreme weather, or other life circumstances that produces an uneven distribution of healthcare resources, including nurses. The individuals in underserved communities lack affordable comprehensive health insurance, have gaps in insurance, or are living in remote areas and unable to access care. Additionally, the lack of basic necessities such as money for food, medications,
The six priority areas are listed as; healthier eating and active living, tobacco free living, reducing harmful alcohol and drug use, improving mental health, preventing violence and injury, and improving sexual and reproductive health. The backbone of the plan is to influence healthy living choices, developing from childhood, throughout life approaching old ages, leading to lessened risks of burden of disease (Department of Health,
Reviewing previously conducted county health assessments and reports is also important in evaluating the trend of the issue over the past few years. Conducting semi-structured individual interviews with key stakeholders from the community, senior Police Officer from Atlanta Police Department, Officer B. Mathew, and the Nurse Manager of Emory Rehabilitation Hospital, Mrs. Elizabeth Thomas, helped to gather data in an informal environment to obtain the most productive responses regarding their professional perspective of the priority community health issues.13 Finally, using creative tools such as windshield survey of the streets of Fulton county and photovoice of the houses and living environment of the certain areas in the county helps to identify and explore the community, its needs, its strengths, and its priority concerns through visual observations and
The overall health scores are based on determinants as well as outcomes. Behavioral determinants include: smoking, binge drinking, drug deaths, obesity, physical inactivity, and high school graduation rate. Community and environmental determinants include: violent crime, occupational fatalities, infectious diseases, children in poverty, and air pollution. Public and health policy determinants include: lack of health insurance, public health funding, and number of adolescent and children immunizations. Clinical care determinants include: accessibility to early prenatal care, availability of primary care physicians and dentists, and preventable hospitalizations. Finally, several different outcomes are used to determine the overall score. Those include: development of diabetes, poor mental and physical health days, disparity in health status, infant mortality, cardiovascular disease, cancer deaths, and premature deaths.
Diagnosing and investigating health problems and health hazards in the community. Informing, educating and empowering the people about health issues. Mobilizing community partnerships to identify and solve health problems. Health assessments also develop policies and plans that support individual and community health efforts. They also enforce laws and regulations that help protect health and ensure safety (Thivierge, 2016). Assessments link people to needed personal health services and assure the provision of health care when otherwise unavailable. Public health assessments can help to assure the public of the competency of the public health and personal health care workforce. These assessments evaluate effectiveness, accessibility and quality of personal and population-based health services (Thivierge,
A community can be defined as a group of people, who live, learn, work and play in an environment at a given time. (Yiu, 2012, p.213) There are many factors that may influence the community’s development and health status. These can include resources available, accessibility, transportation, safety, community needs etc… These influences may combine together to form community strengths and as well as community challenges or weaknesses. As a community health care nurse, it is significant for us to assess and identify these strengths and challenges within the community in order for us to intervene and provide the appropriate needed health care services for the community members. This individual scholarly paper will explore and focus on one challenge issue identified from our group community assessment.
Interventions will also need to be included to meet assessment needs and must also meet at least one of the following criteria. Improve access to healthcare services. Enhance the health of the community. Advance medical or health knowledge. Relieve or reduce the burden of government or other community efforts (Evans, 2011).

...on, race, and political belief, economic or social condition. Improving the poor health of disadvantaged individuals and reducing health gaps is important but not enough to level up health through socioeconomic groups. The objective of tackling health inequalities can be changed to local needs and priorities of a community allowing wide-ranging partnerships of support to be organised. However it needs to be made clear that what can be done to help improve the life chances and health prospects of individuals living in poverty may not come close to bringing their health prospects closer to the average of the rest of the population or prevent the gap living on throughout the generations. Being clear about what is trying to be overcome and achieved needs upmost importance in the development and delivery of policies that will promote health equality across the population.
Initially a profile of a community will be drawn up using research methods, surveys and statistics that will highlight the issues faced by the community. Then a plan...
The five principles of HP include building healthy public policy, creating supportive environments, strengthening community action, developing personal skills, and reorienting health services (McMurray & Clendon 2015). The first principle aims to incorporate health into all public policy decisions beyond the health system so that living and working conditions become conducive to health and equity (Germov, Freij & Richmond 2015). According to McMurray & Clendon (2015), multi-sectoral collaboration is required among different sectors, such as education, industry and social welfare, with the reciprocally influential policies that guide the community health. The second principle emphasises the socio-ecological approach to health that promotes sustainable environment and broader social support systems that encourage a safe and satisfying life (Germov, Freij & Richmond 2015; McMurray & Clendon 2015). This principle requires to acknowledge the significance of conserving the physical or social resources that allow people to maintain health (McMurray & Clendon 2015). The third principle focuses on information and learning opportunities that enable communities to make knowledgeable choices for better health (McMurray & Clendon
The next step is to choose the criteria that we are going to take into consideration. In my opinion, the most important criteria are the following (their order does not indicate their importance):
To reiterate these six components, which are innovation, technical package, communication, management, and political commitment – the community is hand in hand associated with core functions of Public Health. In Public Health the three main core functions are assessment, policy development, and assurance. Assessment is a tool that helps monitors different health and environmental statutes to create, deploy, and identify solutions. It also used a diagnostic tool to investigate health-related problems and different health hazards. Policy development is an act of informing and educating those developed ideas and topics that help the communities and different organizations in their health care efforts. Lastly, assurance utilizes different laws and regulations to help in the aid of protecting the public or environment at risk. It also re-evaluates the laws and regulations to see its effectiveness and its quality (Schneider,
There are numerous public health problems that can be addressed in my Southside of Chicago community. Among the several public health problems facing my Southside of Chicago community there are two that are more urgent. Health education or one might say lack thereof is a problem that needs to be addressed. My community is plagued with many of the residents suffering from high blood pressure, diabetes, and the killer virus known as HIV. In most cases these conditions can be prevented with healthier lifestyles and access to nutritious organic foods. In addition, environmental health is another urgent problem my community is facing. Access to clean, safe water and air is supposed to be a fundamental human right aimed at a healthy environment. Yet, my community consists a waste contaminated beach, numerous deteriorated building that are still occupied, and a countless number of restaurant and stores supplying our residents with services that are endangering their health.
An idea of a healthy community with a safe environment, accessibility to preventive health care services and screenings, education on health promotion that is stress-free and accessible and healthier grocery stores. Characteristics of a healthy community are ideas, activities, and resolutions derived from partnerships of diverse groups, regular self-evaluation that is part of the practice, which is the focus. A diversity of strategies, techniques, documents, and other resources would have to be executed to support and make a healthy community. If an individual 's knowledge of a healthy community is different from his or her belief then community strategy must be put in place in order to begin to achieve the goal of improving health. Community strategy must be supported by the individuals who are working together in order to be successful (Matthew, et al, 2001). In order for a strategy to become effective, a consistent plan should be created with achievable goals within a community. Also individuals that are strong, inspired, and truthful and determined should also carry out the
