Community assessment is a tool, which enables the proper identification of the strengths, weaknesses, opportunities and the threats that are prevailing in a community. It also provides relevant information that assists a health care provider to work in sync with a community and simultaneously address their problems or needs. Community assessment also serves as intervention, where it promotes community participation in the assessment process, ownership of the findings and their engagement with the subsequent outcomes. In order to collect information, few approaches must be religiously cited. These include proper description of the community via geographic maps and census information from the local administration. Local views must also be taken into consideration while collecting the data and this can be done by approaching the local individuals or groups, collecting health views from particular professions like teachers, dentists, pharmacists, social/community workers and religious leaders. After individual surveys, there come the local surveys. The family health assessment is a specialized nursing tool that has three distinct aims: access to the family health needs in collaboration to the family, identification of health services required by the company, proper gain of information for the assessing the accurate need at the community level (Clark, 2015).
Step 3: Assessment of the information
The information collected in step 2 must be analyzed to plan, implement and evaluate the health service. For the proper analysis of the collected information a definitive approach must be undertaken. This includes a rigorous comparative study. Comparison of the selected group of population with a larger population, to ascertain the health issues and the disease rate. This is followed by the comparative data analysis with the previous year data, identifying significant gaps and identification of the positive features as well as the prevailing alarming health problems in the community (Bretan et al., 2013).

Family health is receiving substantial attention in the contemporary decades, following a growing number of unpredicted health issues. Family health assessments have become common techniques within the health care systems across the world to promote good health. Nursing Family assessment and intervention models have been developed in to assists nurses and families to identify the family issues and develop the best.
The Calgary Family Assessment Model (CFAM) is a well-known comprehensive and multidimensional template used by nurses to assess families. CFAM begins by having the nurse visit with the family and gain insight on the family’s functioning at a particular point in time. Interviewing the family allows the nurse to assess and identify potential issues. Furthermore, the CFAM consists of three main assessment categories, known as structural, developmental, and functional. Each of these categories contains several subcategories that allow the nurse to examine all aspects of a family’s functioning. The goal of the CFAM is for the nurse to openly discuss family issues, provide insight to families from an outside perspective and guide them towards their own problem solving tactics. CFAM allows families and nurse to develop a plan of care that is mutually agreed upon. The following paper illustrates a family assessment completed using the CFAM and applies nursing diagnoses and interventions relevant to the family’s current issues (Wright & Leahey, 2013).
In conclusion, the public health nursing profession is an effective tool for providing primary care to the community. The practice existed since the 19th century; however, its significance emerged in the 21st century as a result of changing demographics and global environment. It compliments health care policies through comprehensive nursing assessment, monitoring and intervention through referrals and social support services such as education to promote self-reliance among people at risk. As a result, it is community-based intervention program that involve everyone at risk. It guides assessment of the community’s health status through an elaborate process. For example, the practice develops and prioritizes plans in accordance with research and analysis of health status such as risk factors and assets within the community.
According to Allender, Rector, and Warner (2014), public health is a combination of both an art and a science (2014). The mission of public health nursing is to promote health, prevent disease and ultimately prolong life (Allender et al., 2014). In order for this to occur an assessment must take place. An aggregate or community assessment begins with a collection of data. This includes: the community’s health needs, risks, environmental conditions, financial resources through local census data, and a windshield survey (Allender et al., 2014). Through public health nursing, communities can collectively come together to help promote an overall better health standing.
A community can be defined as a group of people, who live, learn, work and play in an environment at a given time. (Yiu, 2012, p.213) There are many factors that may influence the community’s development and health status. These can include resources available, accessibility, transportation, safety, community needs etc… These influences may combine together to form community strengths and as well as community challenges or weaknesses.
