Reflection Paper Term 4 Clinical
Throughout this year, I’ve learned to be a nurse. From the simple but important tasks of thoroughly washing my hands, to the harder tasks of reading and understanding physician’s orders to the even harder tasks of performing interventions that promote the wellbeing of my residents. This has been a time full of experiences. The most memorable experiences happened in clinical especially this term. A strength I have is that I’m caring. I enjoy taking the time to learn about my residents and to improve their lives. One of my residents was CG, she was admitted due to a hip fracture. People who suffer illnesses, especially acute illnesses, feel their body is betraying them. The ability to ease someone’s anxiety through actively listening, sharing knowledge and providing resources is a gift. This ties into another one of my strengths is that I notice and appreciate my own and other peoples’ humanity and how different everyone is. I'm devoted to understanding and improving situations the best I can without holding myself or others to unattainable standards. We all have biases and judge certain people more unfairly than others but I try to recognize my own biases so I can help as much as possible. Whenever I saw there was an issue on the unit that involved inadequate patient care, I'd politely inform the nurse or simply address it in the moment in a positive manner. For example, a treatment order for a boot was placed on the incorrect foot. There are many reasons why this mistake could have happened. My goal wasn’t for me to be “in the right” or be seen as more competent than another, my goal was to correct the mistake and improve the patient’s outcome. When I was the resource nurse, I knew that one resident, JB, had difficulty transferring the day before. It required multiple student nurses to effectively transfer him. I brought this up to of the members of the physical therapy team earlier in the day. She told me she would check how we were transferring him. And, later that day she did evaluate our transfer technique and showed us how to improve our technique. Ny resident, CG, who mentioned earlier was there for a hip fracture. I noticed her pain medication order was acetaminophen as needed. I mentioned this to the nurse who said that CG was complaining of pain because she was administered the acetaminophen too close to her therapy session. CG had been admitted earlier that week. This was her second therapy session. The next day when her therapists arrived we discussed her pain management. The therapist decided to call the physician to add annorser for schedule pain medication. I knew that during the healing process, she would experience pain but unmanaged pain would hinder yer progress and further set her back. ...diagnoses that cause physical pain, but also problems that can cause emotional, spiritual, and psychosocial trauma. After the implementation of the stated interventions, the patient made physical and emotional progress towards the aforementioned goals. The above goals were not only met, but exceeded expectations of the patient and the nurses who provided care.
In spite of what she has been faced with, she has always had a positive attitude and outlook. She is a woman of strong faith, which I believe has played a big part in getting her through the challenges she has faced. Chronic illness not only affects the patient, but all members of the family as well. For her, it has been a challenge with the expense it has incurred for her. She has health insurance, but some of the medications that she is on are very expensive. In the beginning of her illness, it placed more of a financial strain on her. She was divorced and did not have the best health insurance coverage to begin with. She remarried a couple of years ago and now has better health care insurance through her husband. Not being able to pay for your health care during a difficult time is very hard to deal with. As the nurse, we can offer referrals to agencies that may be able to offer some assistance. Psychosocial care should be incorporated into the routine care of patients with cancer (Jacobsen & Wagner, 2012). Dealing with a chronic illness can also have emotional effects on the patient, as well as other family members. Depression is a commonly incurred condition that coincides with cancer, which can lead to other health and social issues. Overall, I retain three goals for this clinical day: Safely and efficently administer medication, enhance my nursing/CNA skills, and determine how to implement infection control into a health care setting. This week reflects my assigned time to administer medication in a health care setting for the first time, with a resident who retains nearly twenty medications. I except this experience will be a great learning experience, but it will also subsist slightly stressful. With the assistance of my FOR, my goal is to administer all of my resident 's medications without complications. My clinical week was emotional and physically draining this week. I enjoyed being the lead on Thursday because it gave me the opportunity to stop and observe. The nurses and the CNAs were very stressed out, and I clearly saw the effect on the patients. For instance, one of the CNAs asked me to help her with an occupied bed change. I was excited. However, she kept passing a bunch of comments of how hard nursing is and how she did not want to be old. I did not acknowledge any of her comments. Perhaps she thought she could express herself (as a result of her stress) in front the patient since the patient was non verbal and could not understand. I felt very bad. Therapy Analysis The purpose of this paper is to examine the efficacy of my work as a co-therapist during the fifth session with the simulated couple Katy and Michelle. I will discuss our therapy agenda and the goals we hope to attain during the session. It is prudent to begin by giving a brief outline of the couple’s present problem and the patterns of dysfunction that I have identified within their relationship. The main aim of this reflection is to demonstrate that I provided this care. On my first day of week three clinical at 0830, client W and I were on our way to the dinning room and client B asked me to put his jacket on, so I told client W that I would meet him in the dinning room. I believe placing student nurses in the clinical setting is vital in becoming competent nurses. Every experience the student experiences during their placement has an educative nature therefore, it is important for the students to take some time to reflect on these experiences. A specific situation that stood out to me from my clinical experience was that; I didn’t realize I had ignored the patient’s pain until I was later asked by the nurse if the patient was in any pain.

This reflective essay will discuss three skills that I have leant and developed during my placement. The three skills that I will be discussing in this essay are bed-bath, observing a corpse being prepared for mortuary and putting canulla and taking it out. These skills will be discussed in this essay using (Gibb’s, 1988) model. I have chosen to use Gibb’s model because I find this model easier to use and understand to guide me through my reflection process. Moreover, this model will be useful in breaking the new skills that I have developed into a way that I can understand. This model will also enable me to turn my experiences into knowledge that I can refer to in the future when facing same or similar situations. Gibbs model seems to be straightforward compared to the other model which is why I have also chosen it. To abide by the code of conduct of Nursing and Midwifery Council (NMC) names of the real patients in this essay have been changed to respect the confidentiality.
Friday, April 13th, was my last day of clinic and it was a very intense day. I completed my special need patient, an extra patient that may count as an adolescent if Ms. Jones approves it, and screened my board patient. My special need patient was MB, a 39 year old female who suffers from bipolar disorder, depression and stomach ulcers due to medications. My patient is under a physician’s care and is taking her medications. Her vitals were BP: 116/77, R: 20, P: 75, and her intraoral and extraoral exam was within normal limits.
