A. Clinical Strengths & Needs • Strengths: I had a wonderful experience at clinical today and learned a great amount. I feel as I am becoming more confident with working with my resident as well as others. Strengths included demonstrating effective clinical decision-making and safety in practice (objective 5), communicating with the client (objective 2), and implementing strategies to promote, maintain and restore health (objective 4). An example of how I demonstrated objective 5, was when I was feeding a resident that I was unfamiliar with. By continually, explaining the procedure to my resident I am practicing for when I care for other people. All residents/patients need to know what is going on; this can help ease the nerves of some people. This also connects with objective 4, because during this assessment my resident didn’t hear me when I told him I needed him to say “99”. I implemented the strategies of talking louder and looking for facial cues that may demonstrate he didn’t hear me. Not doing an assessment could lead to missing important information such as an abnormal findings. • Needs: Today, I tried to assist my resident from the wheelchair to his bed and it was extremely difficult because he was unable to help me. Fortunately, the CNA was there and showed me her strategy for assisting him from the wheelchair to the bed. She did this by using a gait belt and putting both the residents’ knees in between her knees; while I used the gait belt and had the resident put his knees touching mine. Therefore, something I need to work on is to gain strategies for assisting my resident. I am thinking implementing some of the CNA’s strategy as well as having my resident place his hands on my forearm. As time goes on, I will learn other strategies to better assisting my resident as well as future residents/patients. B. I was able to meet all my objectives; I studied for the med. calculation exam, therefore improving my skills. It’s important to understand and correctly calculate medication to avoid med. error which can be dangerous. I studied very hard for my pathophysiology exam hence I received a good grade on the first exam. And lastly, I am beginning to implemented strategies to aid me in communicating with my resident more; however I want to continue to prefect these skills. Today’s clinical experience truly affected me in multiple ways. I went into this day with an open mind, and was pleased with the patients and the way I was able to conduct myself. This clinical affected me because throughout the day I felt that I experienced many emotions. A few times during my day I did have to fight back tears. I felt I had this emotion because some of the individuals expressed how they wanted to get better in order to get home to their families.
Look back: During my third week clinical experience, I did both computer charting and paper charting (for maternal assessment) with nursing care plan. Besides charting, I reported my significant findings of the mother verbally to the primary nurse.
I realize that I still have many other upcoming experiences during the next two semesters and I am anxiously waiting to learn more about acute care, to prepare even more for the future as a NP and to continue developing a strong, trustful relationship with my patients and my colleagues at school and at the clinical
Utilising John’s model of structured reflection I will reflect on the care I instigated to a patient with complex needs. The patient in question was admitted to the Emergency Assessment Unit for surgical patients then transferred to the ward where I work as a staff nurse.
My experience in mental health clinical was very different from any other clinical I had before. In a mental health clinical setting, I am not only treating client’s mental illnesses, I am also treating their medical problems such as COPD, diabetes, chronic renal failure, etc. Therefore, it is important to prepare for the unexpected events. In this mental health clinical, I learned that the importance of checking on my clients and making sure that they are doing fine by performing a quick head-to toes assessment at the beginning of my shift. I had also learned that client’s mental health illness had a huge impact on their current medical illness.
Reflection is an essential component in the development of professional competencies and critical thinking skills in nursing practice. Reflection in the context of nursing, has been described as a way of exploring an experience in order to look for the prospect of other explanations and alternative methods to doing things. It is through reflection that one can evaluate and identify their strengths and weaknesses to encourage both personal and professional growth and development. This week’s clinical experience has been unlike any other. I went onto the unit knowing that I needed to be more independent and found myself to be both scared and intimidated. However, having the patients I did made my first mother baby clinical an exciting experience. I was able to create connections between what I saw on the unit and the theory we learned in lectures. In addition, I was able to see tricks other nurses on the unit have when providing care, and where others went wrong. Being aware of this enabled me to see the areas of mother baby nursing I understood and areas I need to further research to become a better nurse.
This reflective essay will discuss three skills that I have leant and developed during my placement. The three skills that I will be discussing in this essay are bed-bath, observing a corpse being prepared for mortuary and putting canulla and taking it out. These skills will be discussed in this essay using (Gibb’s, 1988) model. I have chosen to use Gibb’s model because I find this model easier to use and understand to guide me through my reflection process. Moreover, this model will be useful in breaking the new skills that I have developed into a way that I can understand. This model will also enable me to turn my experiences into knowledge that I can refer to in the future when facing same or similar situations. Gibbs model seems to be straightforward compared to the other model which is why I have also chosen it. For the first week we had orientation, which my clinical instructor Maureen went over expected guidelines for clinical, pediatric assessment techniques, how to effectively use SBAR, and ethical issues that can be prevented. I thought all this information was very informative, and will help me with my patient care. I was very frightful of starting this rotation, and I usually go into a clinical with some sort of confidence or act like I have some anyway. During my four-week clinical rotation at Winona Health, I had the opportunity to work with a patient the day following a total knee arthroplasty (TKA) in acute care. I was given the opportunity to work with her for the next 3 weeks, status post-surgery in the outpatient setting. This opportunity allowed me to not only perform a subjective in multiple settings, but it also gave me the opportunity to gain rapport with the patient. It was rewarding to be able to witness the patient’s progress and gain hands on experience treating a TKA in both healthcare settings. This patient interaction was one of the first opportunities I had to create a plan of care, with little help from my clinical instructor, which fit the beliefs and values of the patient.
I want to take part in this Clinical Rotation course because of a statistic that I’ve read in the news stating that almost 50% of all students end up in an entirely different career field than the one which they majored in during college. During this class, I will be exposed to a multitude of different career options that can guide me in choosing the right courses in high school as well as determine what to major in during college. This experience will afford me the opportunity to view a variety of medical careers from a first hand point of view, therefore broadening my understanding of what work fields would best match my interest and skill set.

January of last year, I underwent an evaluation to see if physical therapy would benefit me due severe pain in both shoulders. I found out that my upper and mid back muscles (rhomboids, traps, and rear deltoids) had lengthened and my front deltoids and pectorals had shortened to the point that my shoulders where severely rounded which was creating the pain. I was recommended by both my chiropractor and physician to undergo physical therapy for 3 months at three times a week. I started my treatment at the beginning of February. Throughout the duration of the treatment, I had manipulations performed of on to free and stretch out my chest. I was also given strategies and guidelines to strengthen my back muscles and stretch out my chest during my free time. As a result, I started to workout. I was only able to work out my back muscles during this time and not much else as we did not want to prolong the treatment. After the three-month duration, my posture had significantly improved and my shoulders no longer hurt.
Nursing is not only a career, but a way to give back to the community by enhancing and maintaining the health of other individuals. Throughout my clinical experiences last year, I witnessed kindness and selflessness among many nurses that have impacted and helped shape my future career choices and life immensely.
