Child Observation Report

Goal 1: Jaylan will stabilize his symptoms and behaviors.

Jaylan's brother appeared to be smiling when the QP arrived at their home unannounced and went to get his grandmother from upstairs.
Jaylan's grandmother shared, "she did not know he had to attend orientation on Monday, but his mother said he was going to be home"; however, the QP shared with IIH staff member came to transport him he was not there.
Jaylan and his grandmother confirmed Jaylan was home today, but he is not home now and they do not know where he is.
Jaylan's grandmother commented, "Jaylan told me to tell you he doesn't want to go to Durham Tech over the summer" and the QP informed her he needs to get registration and go through orientation at Durham Tech as well and if he was in place the QP could give him the proper information.

Jaylan's grandmother informed the QP, "Jaylan's friend's grandmother died and he was upset about it because he knew his grandmother well when he would go spend the night at her house."
Jaylan's grandmother stated, "I try to get him up to come to the phone, but he doesn't listen and when we wake him up in the morning he is cranky and can have a bad day; therefore, we just allow him to wake up by himself."
Jaylan's grandmother listened as the QP shared that she will contact Jaylan's mother to schedule a CFT to address concerns.

Jaylan's grandmother received appreciate from the QP for texting back and answering calls in relation to trying to find Jaylan to schedule sessions.
Jaylan made minimal progress with stabilizing his symptoms and behaviors, as indicated by Jaylan eloping from home after his sessions have been schedule by grandmother and not complying with

directions.

Goal 2: Jaylan will prevent an out-of-home placement.

The QP learned that multiple agencies can offer OPT weekly, bi-weekly, or monthly depending on the need of the client.
The agencies identified are Access Family Services, Youth Villages, Diverse Family Services, Upward Changes, ResCare, and Quality Care Solutions.
The QP shared Jaylan will probably be hesitant to participate in OPT plus services because he doesn't like receiving multiple visits per week plus his diagnosis is substance abuse usage.
The Youth Villages staff informed the QP that a referral can be made on their site and Jaylan could be a candidate for YVLifeSet Program.
The Youth Villages staff shared, "Jaylan might not meet all the criteria for HIFI wraparound; however, he does sound like a good candidate for their YVLifeSet Program due to him being out of school, not employed, and looking for ways to improve his lifestyle."
The Youth Village staff stated, YVLifeSet specialist only makes contact with the youth once a week and can be in the program from six to twelve months.
Jaylan's mother reported, "she is available after 6pm" the QP informed her that she will allow the week to play out and get with IIH team members to determine what would be the best day to come together with her and Jaylan to inform them of different options.
Jaylan made minimal progress with preventing out of home placement, as indicated by Jaylan not listening to his mother about staying in place for session and not being motivated to participate in IIH services.
