You, as a team member of Chicken Ranch Casino are being issued uniforms at no cost to you. You are responsible for the maintenance, laundry and care of these items. All uniform items must be kept neat and clean and in good condition at all times. All normal wear and tear will be decided upon by the Wardrobe Department.
REPLACEMENT OF ITEM(S)
• In the event of a lost, destroyed or damaged uniform due to negligence, you are responsible for the cost of the replacement item(s). The cost to replace will be determined by the current cost to the employer. A payroll deduction form is required for replacement of the item(s) and can be completed by contacting the Wardrobe Department.
• In the event of normal wear and tear, items should be exchanged for replacement item(s) through the Wardrobe Department. You will not be charged for the

cost of replacing item(s) due to normal wear and tear.
RETURN OF ITEM(S)
Upon separation of employment from Chicken Ranch Casino, the following terms will be applied:
• All uniform items issued are considered Chicken Ranch Casino owned property and must be returned in good and usable condition to the HR Department no later than 2 days from the last day of employment.

• If the issued uniform is not returned upon separation of employment or the items are returned damaged and unusable, the cost of replacing these items will be withheld from your final paycheck. Normal wear and tear is the only exception to this statement.
CLEANING OF UNIFORMS
• All uniform items have noted washing instructions. General instructions are to wash the item in cold water and may be dried under the low heat setting.
• Items requiring dry cleaning, are to be submitted to the Wardrobe Department. The items will be cleaned and returned to you within a reasonable time period.
NAME TAG
You are being issued a magnetic name tag at no cost to you that must be worn in the upper right quadrant of the uniform at all times.
• Name tags that need to be replaced due to normal wear and tear will be so at the Casino’s

expense.
• Any name tag found to be in need of replacement due to negligence (which includes lost, destroyed, damaged or not returned upon employment separation) will be replaced your expense. The cost to replace will be determined by the current cost to the employer.
• A payroll deduction form is required for replacement of the item(s) and can be completed by contacting the HR Department. The cost to replace will be determined by the current cost to the employer. Once completed the you will receive a name tag.
If you have a pacemaker, you must notify the HR Department immediately as the magnet may pose a health problem.
I thoroughly understand the above Agreement and agree to abide by the indicated terms. Should I fail to return any item(s) listed upon employment separation from Chicken Ranch Casino, my signature authorizes Chicken Ranch Casino to deduct the replacement cost of such item(s) from my final paycheck.
