Being the charge nurse for a day is a great opportunity for us as nursing students to experience how important is to coordinate nursing activities and delegation to improve patient care. It also allows us to develop a more critical thinking by looking others’ fellow students’ actions and learn from what they do.
Personally, it was a new experience that made me feel more confident of my role as a nurse, helping my classmates in anything that they may need, going around into each of the rooms and checking on each patient condition and listening what my classmates were finding during assessment made me forget of the pressure I do sometimes feel when I have my own assigned patients. In this opportunity, I totally forgot of that pressure and I started to recall more information that I was finding regarding the patients’ condition and also, notice things that we as students sometimes omit when we feel overwhelmed.

On the contrary, I felt confident in what I was doing; helping my classmates in anything that they may need like doing wound dressing change, reassessing abnormal vital signs after prompt interventions, keeping track on I&O's, discussing about medication included in patient’s treatment, and suggesting strategies to improve comfort for the patients were some of the occurrences I had during the day.

Growing up, I was never really sure what career would fit best for me. I didn’t know which direction I was going to take and I always ended up pushing it aside because truly, it scared me. Coming into high school, I ended up with multiple injuries – I sprained my left ankle twice and my right ankle once; I also ended up spraining my elbow. This all happened through cheerleading and lacrosse. I was continually going to the hospital and started to realize what my passion was – being in the medical field. My passion for being in the medical field grew even more when my brother married his girlfriend, Yuko. She happened to be a nurse, as well as my other brother who worked as a physical therapist. Though I didn’t know what field I necessarily wanted to go into, and I still am a little conflicted, I knew my heart was in the medical field. One that I’ve specifically looked into recently has been emergency room nurses.
Ever since I was in middle school I dreamed of working in the medical field. I realized nursing was the profession for me when my grandfather became terribly sick with lung cancer during my freshman year of high school. It puzzled me that one of the healthiest and most physically active people I knew could be afflicted by such a damaging disease. After watching my grandfather’s suffering and the pain my entire family felt from his death, I knew I wanted to go into a field to help others that are facing the same challenges. This can lead to occupational burnout which has been linked to suboptimal medical care (Cimiotti, Aiken, Sloane, and Wu, 2012). The impact of sub par health care, results in a higher number of hospital-acquired infections (HAIs) and poor patient outcomes. Implementation of infection control practices and HAI prevention programs are two of the most important aspects of nursing care being utilized to improve patient outcomes. The effect of nurse staffing configurations and nurse-to-patient ratios have also been shown to play a significant role in the relationship between nurses and HAIs.
Nurse staffing is a complex issue. There are many factors which need to be considered when staffing a nursing unit. Evidence shows an association between Registered Nurses (RNs) having decreased workloads and better patient outcomes, including a decrease in patient mortality (Aiken et al., 2011; Needleman et al, 2011; How Lin, 2013; Patrician et al., 2011; Wiltse-Nicely, Sloane, Aiken, 2013). A small percentage of patients expire during their hospitalization as evidence suggests that a portion of these deaths can be attributed to RN staffing levels (Shekelle, 2013). As the reimbursement system for hospitals today emphasizes quality outcomes, this has an increase in the importance of the nurse’s role in patient care (Frith, Anderson, Tseng, & Fong, 2012). The quality of care is effected when there are higher RN-to-patient ratios. Mortality rates can be decreased by 50% or more when there is a lower RN-to-patient ratio. The morale of nursing staff and the hospital’s reputation are effected when there is a large nurse turnover and poor patient outcomes (Martin, 2015; Knudson, 2013). This week’s clinical experience has been unlike any other. I went onto the unit knowing that I needed to be more independent and found myself to be both scared and intimidated. In addition, I was able to see tricks other nurses on the unit have when providing care, and where others went wrong. Being aware of this enabled me to see the areas of mother baby nursing I understood and areas I need to further research to become a better nurse.
Certainly, I began with the goal getting of his vital sign and giving him a bed bath. Fortunately I had practice how to take a vital sign and giving a bed bath before my first clinical at the university skill lab and I had developed this confidence before my clinical tour. That day will be mastering the skill which I had just learned in manikins, and applying in real human with huge safety precaution.
