Intervention As proposed in the literature review, parental adherence to the CDC childhood immunization schedule requires effective provider and parent reminders. The intervention will be personalized to meet the needs of parents and providers of those under the age of 3 in the clinical setting. The AAP recommendations along with the CDC’s immunization schedule established by the ACIP will be the standards for scheduling immunization appointments. The interventions are based on AAP endorsed interventions developed by the Task Force on Community Preventive Services and the CDC’s AFIX approach (AAP, 2014; CDC, 2015). These reminders will be utilized for the parents of patients and electronic reminders will be implemented to assist the providers or APRN’s with vaccination management. While call-reminders have been proven beneficial to ensuring immunization appointments are kept, the provision of more than one form of reminder has been proven more effective and is supported by the AAP and ACIP (CDC, 2015). Parents will be given language appropriate appointments cards for the follow-up immunization date along with a telephone reminder prior to the visit and a text message as applicable. In addition to the use of parental reminders, provider reminders will be implemented by the use electronic prompts within the patient’s health records. These reminders will consist of flagged patient charts that appear when the provider accesses the electronic medical record for the patient (CDC, 2015). With the use of both parental and provider reminders, the likelihood of missed doses of vaccinations decrease. In addition to individual reminders, computer generated reminders will be used to generate daily reminders of those patients within the clinic due for immunizations. The evaluation will be based on the data collected for each individual patient’s immunization adherence and the total adherence percentages for the all the patients in the clinic. CoCASA system data and reports again will be compilated by the designated representative of AFIX and the total percentages will be compared to those statistics at the local and state level. Data for the local level will be obtained from the Robeson County Health Department and the state data for NC from ChildVaxView. ChildVaxView is an interactive system created by the CDC to assist with understanding the data from the National Immunization Survery and interactively create graphs and calculations related to vaccination statistics (CDC, 2016). Utilizing the CoCASA system and the ChildVaxView will provide a data to compare the intervention to the state level for evaluation and to also identify those individual populations requiring further
