At the multidisciplinary meeting, the nurse will collect and assess the information provided by the other disciplines and family members stating that the patient is not at her prior level of functioning and then analyze the information to develop a diagnosis of deconditioning. Next, the nurse identifies outcomes for the patient to get stronger, achieve prior level of function, have activities of daily living (ADL’s) met in a safe environment by planning for home health, equipment, and 24/7 supervision through family or placement in a facility. This will be implemented by coordinating delivery of a walker and a 3 in 1 chair prior to discharge to daughter’s home with the home health agency nurse, physical therapist, and aide scheduled to start that day. In a week, the nurse evaluates that outcomes are being met by following up with patient, daughter, and home health agency evaluating that the patient is getting stronger, ADL’s are being met, and will soon be able to return to living independently. To achieve these standards of practice, every nurse should be aware of her own nurse practice act to ensure to be functioning with in the laws of the nurse’s state and to ensure the best outcomes and safety of the patients. Health information records include patient histories, lab results, x-rays, clinical information, and notes. The data can be analyzed to see how a patient’s health might have changed. I took interest in Health Information Management when it was brought to my attention by a doctor. He told me that is a very interesting field and it is in high demand as they have more jobs than people to fill them. I went home, researched it and now here I am making my entry into the field.

The nature of the work is very similar for the C.N.A. and L.P.N. A C.N.A. work includes performing routine tasks under the supervision of nursing staff. They answer call bells, deliver messages, serve meals, make beds, and help patients eat, dress, and bathe. Aides also provide skin care to patients, take pulse, temperature, respiration, and blood pressure and help patients get in and out of bed and walk. They also escort patients to operating rooms, exam rooms, keep patient rooms neat, set up equipment, or store and move supplies. Aides observe patient’s physical, mental, and emotional condition and report any change to the R.N. Likewise the L.P.N. provides basic bedside care. They take vital signs such as temperature, blood pressure, restorations, and pulse. They also treat bedsores, prepare and give injections and enemas, apply dressings, apply ice packs and insert catheters. L.P.N.’s observe patients and report adverse reactions to medications or treatments to the R.N. or the doctor. They help patients with bathing, dressing, and personal hygiene, and care for their emotional needs.

This continues to allow vulnerability in access to protected patient information and potential for data breaches. Breaches can occur due to human error, improper disposal, hacking of information, and numerous other reasons. One breach occurred due to both human error and improper disposal in a regional hospital located in Pennsylvania.
