On May 5, 2016 I was dispatched to 161 SE. 1st Ave. for domestic disturbance. The disturbance was reported by inmate at the jail. When I arrived I made contact with Gage DeWald. Deputy David Melcher made contact with Gabrielle Hernandez. DeWald stated that when he got home Hernandez was talking on a phone to Dakota Gray who was in the jail. This upset him and he told Hernandez that she needed to leave. He was trying to get her out the back door. He stated that she had a knife in her hand. DeWald also stated that she bit him in the fore arm on his right arm. DeWald stated that Hernandez has not been living there but stayed last night to watch their infant daughter. DeWald stated that he had kicked Hernandez out of his house once before. He
Gary Dougherty was paroled from Northeast Correctional Complex on 11/15/2017. Mr. Dougherty has a Tennessee Sentence of Attempted First Degree Murder and is currently under minimum supervision level. Mr. Dougherty was paroled to Steps Halfway House. On 04/16/18, Case Manager Ron Stephens advised me that Mr. Dougherty was discharged from Steps for several rule violations. Mr. Stephens advised that since Mr. Dougherty had been at Steps he has failed three drug screens, offered drugs to another resident, ask residents for clean urine, brought a prostitute in the house, and threatened a resident.
On 06/29/2016 at approximately 2106 Arlington Police Department were dispatched to a domestic violence call at 18112 31st AVE NE, Arlington, WA 98223. Involved parties are Leonard, Andreu A. (09/25/1999) and Leonard, Frank J. (11/23/1977).

A serious case review is a local enquiry into the death or serious injury of a child, where abuse or neglect are known or suspected. These are conducted by the Local Safeguarding Children Boards; with the main focus being on what lessons can be learnt locally to prevent this from happening again (Brandon, Bailey, Belderson, 2010). In this textual analysis we will be looking back at previous case reviews including Jasmine Beckford and Baby P. We will then look at what recommendations have been made and use the Peka case to see weather we have learned from our previous mistakes or are we still in the same position now as we where then.
It was a normal evening in the Maloney home. Mrs. Mary Maloney sat sewing, while waiting for her husband to return home after an involved day as a police officer. Around 5 o’clock Mr. Maloney returns home with shocking news and… Bang! …a leg of lamb hit over his head and Mr. Maloney falls to the ground dead. All evidence and theories, point to Mrs. Maloney being the killer of her husband, but why? Mrs. Maloney did not kill her husband out of anger after the recent marriage incident, but she did it as a result of mental anguish, self defense and trauma inflicted upon her by her husband. All these events explain exactly why Mrs. Maloney murdered her husband out of reasonable measures.
In summary, on 06/05/16 at 1741 hours Ofc. C. Zepeda #253, Ofc. E. Vera #289 and I were dispatched to 3215 S Central Ave. in regards to a disturbance.
Shroyer took Fiona McFadden into DCS state custody. A DCS Temporary Custody Notice and DCS Notice to Inform were served on Monica Katich and Christopher McFadden. This Investigator approached Officer Cambell who had taken custody of the folding knife located in the apartment and inquired about the length. Officer Cambell reported that the knife was 8 inches long and 4 inches in blade length. Officer Gomez approached this Investigator about having received the information of Monica's father and step-mother. This Investigator spoke with Monica in the back of the Phoenix Police vehicle to obtain her family's information. Monica inquired what was "going on" with Fiona. I informed Monica that the State of Arizona and DCS were taking custody of Fiona. Monica became upset and asked why she could not stay with her boyfriend Christopher McFadden (DOB: 11-24-87) who is the biological father to Fiona and also resides in the apartment. According to Monica, Christopher works at Well Fargo and would be home around 2:30 AM. Monica was advised that Christopher was not appropriate for placement, and the process that was already explained to her by Investigator V. Shroyer was again explained to her by this Investigator. Monica stated that she was in the prior care of Indiana CPS when she was a child and that there had been domestic violence from her biological mother. On 01/30/16 I arrived at 3810 Driskill Drive in reference to a disturbance involving a North Little Rock police officer. Upon arrival I made contact with the complainant, Aimee Stepp. Mrs. Stepp said that her daughter, Katherine Kuykendall, and her ex-husband, Terry Kuykendall, had been in a physical altercation and that she, Mrs. Stepp, had an audio recording of the event on her cellular telephone. Mrs. Stepp also stated that her daughter had just sent her a text message stating that she was ok.
On 01-28-2017 at 1002 hours I was dispatched to 305 Bartlett apartment A in reference to multiple subjects in the residence.
Upon my arrival, I spoke with Natalie Stovall who stated that her and her boyfriend had gotten into an altercation but he left the residence prior to our arrival. Natalie stated that she did not wish to pursue any charges regarding the assault. I then had dispatch run Natalie’s driver licenses to make certain she was clear from any warrants. Dispatch advised that Natalie had two warrants out of Dallas County.
INVESTIGATION: At 0825 hours, this writer along with Sgt. Murchison responded to the command post located at M-1 place of abode. Upon arrival on the scene we were met by officer Hamrick of the Metropolitan Police Department, 4th District. This writer asked Officer Hamrick what had been done prior to Youth And Family Services Division responding to the scene. Officer Hamrick told this writer the inside and outside premised were checked with negative results
...d a phone call from Deputy Ross who was located at Covenant Emergency Room with Deputy Lawless. Deputy Ross advised the two walk-in patients the emergency room received where involved with the incident that occurred at the Alamo Trailer Park. Deputy Ross advised both individuals sustained severe injuries. Deputy Ross advised one of the patients, later identified as Jaime Ornelas-Gonzales sustained a severe laceration on his right bicep, a possible stab wound to the base of his skull, and several other small cuts on his body. Deputy Ross advised the second patient, later identified I told Deputy Ross to get the information from the patients and begin photographing the injuries, he agreed (photos attached to the report). I also asked Deputy Ross to stay at the emergency room with the patients until officers from this department could speak to them, he agreed.
On 03/17/17 I was contacted by Deputy Garrison in reference to a Use of Force that occurred on a traffic stop in the 8100 Block of Highway 161. I responded to Deputy Garrison’s location and made contact with him, Deputy Blocker and Deputy Crim.
In summary, on 03/11/16 at 1516 hours I was assigned to a Domestic Incident at the Cicero Police Station.
On 04/23/2018, I, Dillon Dickerson, badge #155, was working as a Patrol Officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately 2313 hours dispatch received a call from a concerned friend of a student names Olivia Bradely later identified by Shocker ID A776S542. Dispatch gathered information from the reporting party and then called the squad room. Dispatch advised they were getting a call of a possible suicidal student at Shocker Hall. Dispatch advised the female who we needed to check on was in Shocker Hall room D3090. I informed dispatch via radio I would be heading to Shocker Hall. Dispatch advised the Assistant Residence life Coordinator Jesus Gomez Llanos Tirado would be waiting
