Shoulder Injury Diagnosis and Treatment
Population:
Vincent is a 23 year old male in his first year of grad school for DPT. He started wrestling in the first grade and continued into college. Up until his junior year of college he wrestled for the University . From all the stresses and pressure constantly on his shoulder, at some point a shoulder injury was bound to happen. He is very flexible, but from being placed in unwanted positions with limbs being stretched at rates they should not be, injury was done. His ligaments were stretched, which increased his risk of injury.
Gender: Dislocation occurs in 70% of males (VMC).
Age: Shoulder dislocation is a common consequence of athletic activity in the younger population, occurring in up to 7% of the population. Rotator cuff tears are more common in patients 35 years or older (VMC).
Mechanism: 95% of anterior shoulder dislocations are due to traumatic injury, most commonly resulting from a fall. Those who play certain sports, or who undertake activities with high physical demands are at increased risk. It is seen in athletes who use their upper body for much of the sport. For example, weightlifters, wrestlers, baseball players, swimmers, volleyball players, and tennis players are all susceptible. It is especially common in tennis and volleyball players because of the forces on their shoulder during the cocking and acceleration phases of a serve. The area of the labrum where the SLAP tear occurs is susceptible to injury because it is an area of relatively poor vascularity compared to the other parts (VMC).
Location: 47.7% of dislocations occur at home, while 34.5% occur at sites of sports or recreation (VMC).

Many factors like patients age, symptoms, size of the tear, and nature of onset (traumatic or degenerative) are responsible for determining the prognosis of the physical therapy treatment 9. Since it’s a partial thickness tear of the rotator cuff, the non-operative treatment is reasonable unlike, a full thickness tear where surgery will be required to treat the patient 9. The patient shows symptoms of moderate irritability without significant functional deficit which makes him a good candidate for conservative treatment 10. In this case, with three weeks of physical therapy the patient has showed a decrease in pain and reported it as 4/10 on pain rating scale and showed improvements in abduction (140̊) and external rotation (65̊). The most widely publicized incident of ACL damage has come from Theresa Edwards who was a top female athlete. She was a basketball player who went to the limit with sports. She went beyond her capability and her ligament couldn’t withstand the pressure and snapped. She is not the only but just one example of many who have suffered this same problem. The most common knee injury in sports is damage to the anterior cruciate ligament (ACL) through tears or sprains. “They occur in high demand sports that involve planting and cutting, jumping with a poor landing, and stopping immediately or changing directions” (University of Colorado Hospital). Its function involves controlling the back and forth motion of the knee, preventing the tibia from sliding out in front of the femur, and providing rational stability to the knee. Interestingly, women are more prone to ACL injuries than men. The occurrence is four to six times greater in female athletes.

I speak with him regularly about trends in the psychological effects of injuries, as well as injury trends per sports and per football position. People have created a hectic and busy world, that includes careers and daily activities that require physical activity. Sometimes, people can surpass their current limits and form new boundaries; however, other times people are not so fortunate. These unfortunate times often lead to injury, including workplace accidents, sporting incidents, disease afflictions, as well as others; any or all of which could bring about the need of rehabilitation services. Many of these require physical therapy, which includes assisting injured or otherwise impaired patients as they recover to their pre-injury status or to recover as much as is physically possible. The field of physical therapy is a choice career for those who enjoy helping people recover from injury, and the following text will provide reason for choosing this profession.
Patient returned the next day still complaining of pain. The PT applied heat, then initiated the exercise program, but the patient could not perform theem to same extent as previously, secondary to pain. Therefore, the PT told the patient to schedule an appointment with his physician. The past two and a half weeks have been very productive and I continue to make progress in continuing to regain my tissue mobility. The medication in conjunction with the work hardening program is working and the restriction in my shoulder appears to be lessening in small increments weekly. I am trying to be patient and had one bad session from me trying to push too hard too fast in an effort to move through my restriction, but I now realize that slow and steady will win this race. Additionally, my sleep periods are improving. I am successfully working two hours per day from home without flare-up of my shoulder and my at home work hardening sessions have increased to nearly four hours.
