A. A woman and her 8 month old son come into the hospital. The boy is very thin and wasted looking and the mother tells you he hasn’t been growing at all in the recent weeks. Through interviewing the woman, you discover that their family has come on hard times and in order to cut down on costs, she and her husband have been diluting the boy’s formula and have not yet introduced him to solid foods.
1. What condition is the little boy likely suffering from?
The boy is suffering from Marasmus which is a chronic malnutrition caused by the deficiency in both the energy and the proteins. Diluting the formula causes the dilution of the nutrients it contains as well. So feeding your baby a diluted formula would cause him to not get enough protein, carbohydrates, fats, vitamins and minerals, which are essential for the normal growth and development for the baby. Your boy is in the early stage of Marasmus which is a chronic malnutrition. When you changed the concentration of the baby’s formula it affected him to not receive enough nutrition in his diet which is causing him to get stop growing and get thinner. If it continues he might become more deficient in many of the essential nutrition such as iron which can cause him to have anemia. He also lacks calcium in his diet which will cause him to have weak bone and teeth. Also if you use tap water to dilute the formula it might cause the baby to have fluoride overdose which can damage his tooth buds and have fluorosis which can make his teeth emerge as stained black. There are many more essential nutrients that he might be lacking in his body which will eventually lead him to be severely sick and death can also
My virtual child’s name is Dominic. He is a white male of average height and weight with blonde hair, green eyes and pale skin. Cognitively he seems younger than his chronological age up until sixth grade. Physically Dominic has always seemed older than his chronological age. The first few months of Dominic’s life were rough at times. I decided from the beginning to feed with formula, and I would respond to different types of cries. At three months he would cry after each bottle and have frequent diarrhea, we then decided to switch to a different formula rather than introducing food (My Virtual Child).
The first year of a baby’s life is a time of rapid changes and figuring out who to trust in the world. That first year many things happen that are very important to the future of the infant. This point in life is a time of “rapid physical and nervous system development, accomplishments that ensure an infant’s survival and ability to cope with its world” (Dacey et al., 2009). Babies rapidly gain weight in the first year so nutrition is a very important part of the development process. They need a good diet consisting of “carbohydrates, proteins, minerals, and vitamins” (Dacey et al., 2009). Proper nutrition ensures the correct development of newborns survival. “Having survived prenatal hazards, newborns come equipped with automatic responses ideally suited for survival” (Myers, 2008). The first year of a baby’s life is very important in ensuring a successful journey heading into their future.
1. Normal. It is normal for a newborn baby to have watery or loose stools in the early months after birth when she is only receiving mother's milk. This stool is often yellowish-brown in color and is similar to mustard in consistency. It is also common to see white particles that look like seeds in the stool. However, there are many normal variations in the stools among breastfed babies, but soon you should see what looks like normal stool for your baby.
For example the present study shows that the feeding time in babies fed with the syringe was 10ml/1.25min compared with 10ml/2.08 min without palatal obturation for cup and spoon fed babies at 6 weeks. This demonstrates that babies fed with either the syringe or the cup and spoon showed a faster feeding time than those fed with the obturator. The syringe feeding also indicated a greater volume of feed and faster feeding time. This study also shows less spill and regurgitation which is most likely a result of the tip of the syringe entering the oral cavity away from the cleft. Since the volume of the milk is measured, a more efficient delivery of the milk is possible, resulting in fewer spills. For example, a problem rests in the use of tablets and capsules that lack dosing flexibility. Tablets and capsules are usually not created or dosed for children and thus adult tablets and capsules are modified by compounding. However by grinding the tablet and sprinkling it into fluid or a food vehicle usually results in dosing variability from one administration to the next. Taste is also a major problem for pediatrics. Alot of the time children do not like the taste of non-masked formulations. There are many physiological and pharmacokinetic differences between adults and children. As technology advances, it will become easier to research and create pediatric formulations.
Many children suffer from dehydration and become symptomatic quickly. In an article by Spandorfer, Alessandrini, Joffe, Localio, and Shaw, it was found that approximately 10% of children admitted suffer from dehydration due to gastroenteritis (2005). Oral hydration cannot always be achieved especially if the child is vomiting as well as suffering from diarrhea, which leads parents to take their children to the hospital for care. Parents may need education and emotional support in the prevention of dehydration in children.
