Reflection is the thing that we bring to an experience is fundamental to our understanding of what happens. This is impacted by our past, our future and our present world-sees. In nursing, it reflects the attitude, personality, decision-making and ethics when dealing with sick people. A Cherima (2007) point out that reflection journal is a useful tool for promoting reflection and learning process for nurses. In this assignment, I am going to reflect on one of the clinical situations that had happened during my clinical placement at the surgical orthopedic ward. The incident that I am going to reflect is maintaining patient’s safety in preventing risk of fall during the hospitalization. It is important to prevent the patient from fall because it may further impact the patient’s wellbeing. For instance, the patient might experience fractures from falls. Edwards et al. The objective is to reflect on the clinical event that I had experienced by discussing the issue, analyse the situation and give recommendations for future use with the guide of reflective model framework. I have decided to choose Gibbs’ model of reflection (1988) to reflect the incidence. Gibbs’ model (1988) is commonly used in the health profession. It has 6 stages which include description, feelings, evaluation, analysis, conclusion and action plan. In each stage it has questions that need to be considered and I am going to use it as a guide in reflecting the event. See appendix 1 for the cycle of stages of Gibbs’ model (1988) details. The purpose of this essay is to reflect upon the teamwork and communication within the multi- disciplinary environment of the theatre suite. I will follow a patient on their journey along the Patient pathway, from their arrival in the anaesthetic room, through to them being handed over to the recovery staff. The patient pathway is described by the Department of Health (2007) as a timeline, on which every event relating to treatment can be entered. Events such as consultations, diagnosis, treatment, medication, diet, assessment and discharge can be mapped on this timeline. In this essay, I will use the Johns 1994 mode of reflection. The purpose of reflection, as stated by Johns (1994) is to promote desirable practice through the practitioners understanding and learning about their lived experiences. Patient trust and confidentiality will be maintained throughout, the patient referred to as Mrs X and any details will be improvised.
The Quality and Education for Nurses (QSEN) project has set several goals for future nurses to meet in terms of knowledge, skills, and attitude (KSAs), one of which is safety (2014). The definition of safety according to QSEN is minimizing risk of harm to patients through system effectiveness and individual performance (QSEN, 2014). Since falls are such a huge occurrence in health care, preventing falls is critical for patient safety. The Joint Commission (2011) has also noted fall prevention as a National Safety Patient Goal (NPSG) 09.02.01 requiring hospitals to reduce the risk of harm resulting from falls.
Critical incidents are snapshots of something that happens to a patient, their family or nurse. It may be something positive, or it could be a situation where someone has suffered in some way (Rich & Parker 2001). Reflection and analysis of critical incidents is widely regarded as a valuable learning tool for nurses. The practice requires us to explore our actions and feelings and examine evidence-based literature, thus bridging the gap between theory and practice (Bailey 1995). It also affords us the opportunity of changing our way of thinking or practicing, for when we reflect on an incident we can learn valuable lessons from what did and did not work. When I was working as a bedside nurse in the Emergency Department, in one of my duties I was not satisfied with the treatment plan made by a resident doctor for XYZ patient. He entered intravenous KCL (potassium chloride) for the patient. The purpose of that medication and its dose for that patient was not clear to me. I assessed patient history and came to know that a middle aged patient came with the complaint of loose bowel movements, vomiting, and generalized weakness. His GCS (Glasgow comma scale) was 15/15, looked pale but was vitally stable. The main aim of this reflection is to demonstrate that I provided this care. During my training as a student nurse, I have been involved with many patients with complex needs of a with the support of a mentor, however this was, primarily, the first time since qualified and on completion of my registration that I became responsible and accountable for my practice.
Developing confidence, and competence is a challenge faced by novice nurses (Morrell & Ridgway, 2014). Over the course of my nursing degree developing, and maintaining confidence in my clinical practice has always been a personal challenge. During my preceptorship placement, I have the opportunity to continue to cultivate my confidence, and prepare to begin my practice as an independent graduate nurse. In the reflection, I will discuss how I have gradually become a confident practitioner through my experiences in my clinical placement, and especially those in my preceptorship placement.

Leaders must engage the nurses in applying the evidence-based therapies so that they can ensure good safety for the patients. Nurses are important in ensuring advocacy, education and the management of the facility environment. The nurses will therefore apply various necessary interventions that guarantee safe environments for the patients and the nurses.
In this reflective essay, I will be using the Driscoll’s model of reflection to talk about how my knowledge of quality nursing care has improved since the commencement of this module. Quality nursing care has helped me develop various nursing strategies that will guide me in my first placement and throughout my career in nursing. Furthermore, it has taught me about communicating effectively with patients, I have learnt about verbal communication such as paraphrasing when communicating with patients to ensure that what said is properly understood. I have learnt not to make assumptions about patients and putting them in the middle of their care, taking into account their preferences.

The names of people have been changed to ensure confidentiality Nursing and Midwifery Council (NMC, 2008). Gibbs (1988) cited in Jasper (2013) will be used as the reflective model because it is simple and, easy to understand. Through the model’s six key stages I will describe my experience and how I maintained dignity while giving personal care to patients in the community. Writing a reflective account makes one relive their thoughts and, feelings and make appropriate changes when required (Howaston-Jones, 2013).
This reflective essay will discuss three skills that I have leant and developed during my placement. The three skills that I will be discussing in this essay are bed-bath, observing a corpse being prepared for mortuary and putting canulla and taking it out. These skills will be discussed in this essay using (Gibb’s, 1988) model. I have chosen to use Gibb’s model because I find this model easier to use and understand to guide me through my reflection process. Moreover, this model will be useful in breaking the new skills that I have developed into a way that I can understand. This model will also enable me to turn my experiences into knowledge that I can refer to in the future when facing same or similar situations. Gibbs model seems to be straightforward compared to the other model which is why I have also chosen it. To abide by the code of conduct of Nursing and Midwifery Council (NMC) names of the real patients in this essay have been changed to respect the confidentiality.
This reflective essay will lay emphasis on one of the learning needs I have developed during my two week taster placement in hospital. Reflection helps an individual build upon their skills and makes room for self-criticism as he or she can contemplate upon actions and make relevant changes (Taylor, 2000). I will be applying the “What”, “So what” and “Now what” model of reflection by Driscoll (2000) in this piece of work because it is a more coherent and comprehensible approach to follow when writing a reflective account and is also an easier guide to writing reflections. The learning need I chose to reflect on from my learning plan is having a better understanding of diabetes and the 6 basic medications used in treating the condition since it is a common illness on the ward I am have been allocated on for my first placement. The timescale set for achieving this objective was by the end of my two week placement that is from 27th January to 9th February and I achieved it with the help of a host of factors. In this assignment, all the names of the patients and wards have been omitted and indicated with letters and numbers for confidentiality reasons as stated in the Nursing and Midwifery Council (NMC) code of conduct (2008). The paper will primarily touch on the type 2 diabetes and furthermore on the achievement on my learning need.
My senior capstone unveiled more obstacles than I could have ever anticipated. It all started off rocky having to adjust to a new teacher with higher standards and expectations for my nursing class. I also challenged myself to go beyond my comfort zone and work with those whom I don’t normally work with, as well as learn about a topic I wasn’t necessarily comfortable with. I experienced first hand conflicts with my group members, advisor, and the overall project.
In this assignment I will reflect on a situation that happened during my first hospital placement. Reflection is a process of making sense out of all life experiences in general and nursing practice in particular (Taylor B 2004). It seeks to describe, analyse, evaluate and therefore inform my learning experience in practice. I have chosen to make sense out of that experience by employing Gibbs’ model (1998) reflective cycle. I will explore the bathing practice used in hospital, and how best the bathing process as a pleasant experience. I knew this practice placement experience would be different from the community placement I had previously experienced. In this clinical setting, as a student I was conditioned to bathe patients first thing every morning. It seemed this was a ritual routine which ward wanted carried out before patients had their breakfast. The system worked well when there were few assisted washes to be done.

We are just starting our clinical rotations at KGH. At that time, our instructors assigned us one patient to do our nursing care. The goals for the day are to do our head to toe assessments and be able to chart our findings.
Reflection is an important part of development and growth for all medical professionals. It gives us a chance to assess our assumptions and make active change in our behaviour towards both our patients and our team.
