Functional Assessment The functional family assessment shows how individuals behave in relation to one another. It shows the in-the-moment aspect of a family’s life. There are two basic aspects of family functioning: instrumental and expressive (Wright and Leahey, 2013). Instrumental Functioning Instrumental functioning of the family shows the routine activities of daily living. This area is important for families with health problems in particular (Wright and Leahey, 2013). The father, DM, wakes up around five thirty in the morning to be to work by seven o’clock. EM eats three meals per day and has a regular schedule of going to school five days per week. On the weekend, EM participates in family activities that CM and DM plan for the family. He goes to friends’ houses and enjoys helping DM when it comes to anything and everything he does around the house. EM goes to bed between 6pm and 7pm and does not regularly take any medication. The middle son, JM, wakes up around 6:15am when CM wakes up his older brother. JM goes to preschool three times per week for a half day at the local Lutheran church that the family is part of. JM eats three meals per day and has a regular schedule. On the weekends JM also participates in family activities. JM goes to bed with EM between 6pm and 7pm each night and does not regularly take any medication. The youngest daughter, AM, wakes up once through the night and is normally up between 5:30am and 6am and then takes a long nap around ten in the morning and short naps throughout the day. AM is still breast-feeding and has not yet been introduced to food. AM is currently cutting teeth so she is abnormally grumpy and does not take any medications regularly. This is another strength of the family. They are optimists and always think about the positive aspects of every situation. The most evident weakness is the busyness. DM isn’t home often and it puts more stress on CM to do everything for the family. CM does all of the day-to-day chores while still caring for all of their children during the weekdays when DM isn’t home. This isn’t easy on DM to not be home as often as he is. He, from time to time, feels like his is missing out on a lot of the things happening in his life. Another weakness is that the parenting is mostly left up to the mother. This cannot only be stressful for both parents but also stressful for the children. Not having help from extended family is also a weakness for the M family. They get a large amount of help from their nanny and they close friend group but not having the family help with the children is hard. According to smith and Hamon (2012), Families are considered as a whole in society. However, they believed that couples have many components in which makes up the family, if one component is missing, the family as a whole can get unbalance (Smith & Hamon, 2012). In the Brice’s family, communication was the component that was missing. The couple was not able to communicate their differences, which was what caused Carolyn and David to verbally insult each other. Smith and Hamon (2012), also explain that a person who expresses his or her feeling is considered as someone who is breaking the functions of their family system; especially if the person is focusing on the individual who is causing the problem, rather than the problem itself. In the Brice family, Carolyn could be considered the one that cause the dysfunction in the family structure because she was focusing on David as the problem of their marriage, rather than focusing of the elements that are causing their problems. The Calgary Family Assessment Model (CFAM) is a well-known comprehensive and multidimensional template used by nurses to assess families. CFAM begins by having the nurse visit with the family and gain insight on the family’s functioning at a particular point in time. Interviewing the family allows the nurse to assess and identify potential issues. Furthermore, the CFAM consists of three main assessment categories, known as structural, developmental, and functional. Each of these categories contains several subcategories that allow the nurse to examine all aspects of a family’s functioning. The goal of the CFAM is for the nurse to openly discuss family issues, provide insight to families from an outside perspective and guide them towards their own problem solving tactics. CFAM allows families and nurse to develop a plan of care that is mutually agreed upon. The following paper illustrates a family assessment completed using the CFAM and applies nursing diagnoses and interventions relevant to the family’s current issues (Wright & Leahey, 2013).

Therefore, it is important for health care providers to develop models of care based on an understanding what families are going through (Eggenberger, Meiers, Krumwiede, Bliesmer, & Earle, 2011). The family I chose to interview is in the middle of a transition in family dynamics. I used the family as a system approach as well as a structure-function theoretical framework to the effects of the changes in dynamic function. Additionally, the combinations of genogram, ecomap, adaptations of the Friedman Family Assessment model as well as Wright & Leahey’s 15 minute family interview were utilized.
