Peritoneal dialysis is an alternative to hemodialysis that allows patients with kidney disease the ability to be flexible and maintain a high quality of life, while receiving dialysis. Continuous ambulatory peritoneal dialysis (CAPD), intermittent peritoneal dialysis (IPD), and continuous cycling peritoneal dialysis (CCPD) are some of the options available to these patients and although they are all different they operate on the same premise. A permanent, indwelling catheter will be inserted into the peritoneum and using aseptic technique, the indwelling catheter is hooked up to a large bag of dialysate. The fluid is then infused into the peritoneal cavity either by machine or by gravity. Through diffusion and osmosis extra fluid and waste products enter the dialysate fluid and are removed from the body when the fluid is drained. The process involves a specific fill, dwell and drain time which altogether equals one exchange. The choice of fluid type, treatment, and exchange time are based on the specific needs of the patient (Lippincott, Williams and Wilkins, 2014). In order to be able to utilize this type of dialysis a patient must not have extensive abdominal adhesions, or multiple abdominal surgeries, and they must not have a cognitive deficit that would impair their ability to carry out the set procedures necessary to safely perform the treatment (Mendelssohn, et al., 2009). There are many benefits to this method of dialysis, but there are also several serious risks involved such as peritonitis, severe protein loss, fluid volume overload, catheter site infections and abdominal hernias. Dialysis is used when a person’s kidneys have lost 85-95% of their functioning ability. Over 100,000 people in the US are staying alive due to kidney dialysis. Kidney dialysis is also called renal dialysis. There are two types of kidney dialysis: peritoneal and hemodialysis. Both types of dialysis remove waste, salt and extra water, which builds up in the body. Dialysis not only cleanses impurities out of the blood, it also maintains blood pressure and makes sure to keep healthy levels of sodium, bicarbonate and potassium in the body. (Lewin, 2010)
Currently there are two types of renal replacement therapy. The original dialysis which we called it hemodialysis used for patients with chronic renal failure, needs the patients to come to hospital 2-3 times per week. This type of dialysis called the intermittent hemodialysis .However the intermittent hemodialysis is difficult to do it in the intensive care population with acute renal failure because of the hemodynamic instability and those type of patients usually are the most sick and critically ill patients within the intensive care units and have multiorgan dysfunctions, so medically is too difficult to do for them intermittent hemodialysis. In addition intermittent hemodialysis will increase the mortality and morbidity among them. Dialysis is a process by which excess waste and water is removed from the blood to maintain a balanced proportion of contents/nutrients. Dialysis is done by using different dialysis machines which are usually very costly and sensitive. The duty to maintain a constant proportion of contents in the blood is usually done by kidneys but when they are not functioning properly the procedure of dialysis used. Therefore it is a very important and beneficial process for people whose kidneys are not functioning properly or who cannot undergo a kidney transplant due to various reasons for example blood type or reactive immune system. To the majority of Americans, dialysis is a confusing process that they will hopefully never have endure, but for hundreds of thousands people, it is a daily fact of life. According to US News and World Report, “In the United States, almost 400,000 people undergo dialysis every year.” (Gordon, 2012, para. 4) Their lives revolve around receiving dialysis three days a week for three to four hours per treatment, usually at an outpatient clinic. While dialysis can prolong the end stage renal disease (ESRD) patients’ life, the three day a week trudge to this clinic can feel like and endless cycle of discomfort and inconvenience. Fortunately the ESRD patient has more than one option, namely, peritoneal dialysis.

The University of Pennsylvania Hospital in Philadelphia has a Kidney Dialysis Machine which can serve the functions of a healthy kidney, for patients with kidney disease. Kidney disease occurs when the organs no longer function and waste products begin to build up within the body. This disease can lead to anemia, high blood pressure, and nerve damage. In the process of kidney dialysis, there are catheters which are connected to our veins through a vein graft which is attached to the tubes of the Dialysis Machine. In essence, blood is simply being pumped into the machine where waste products are filtered out, and then the ‘clean’ blood is pumped back into our body through a vein graft. There are ten possible candidates with chronic kidney disease who could potentially be granted access to the Kidney Dialysis Machine. There are only two time slots, so the candidates will be assessed according to condition and other factors as to who is allowed to use the machine. I believe that Steve should be one of the two patients who are granted access to the Kidney Dialysis Machine at University of Pennsylvania Hospital.
