At this point in time the Lees were demonstrating behaviors that the health care provides wished would of showed up sooner in Lia’s health condition. Two years later, the Lees are providing Lia with the adequate care that she needs. The Lees are making sure Lia is eating and is bathed every night. They make sure that she is groomed and dressed appropriately. Since Lia is not considered an epileptic any more there is no need for a drug regimen, but the Lees continue to treat her with Hmong medicine such as teas. When it was time for Lia to receive her bath Foua would join her, because at the end of it all she would be soaking wet. Membership Services (MSD) at Kaiser Permanente used to be a modest department of sixty staff. However, over the past few years the department has doubled in size, creating minor departmental reorganization. In addition the increase of departmental staffing, several challenges became apparent. The changes included primary job function, as well as the introduction of new network system software which slowed down the processes of other departments. These departments included Claims (who pay the bills for service providers outside of the Kaiser Permanente network), and Patient Business Services (who send invoices to members for services received within Kaiser Permanente). Due to the unforeseen challenges created by the system upgrade, it was decided that MSD would process the calls for both of the affected departments. Unfortunately, this created a catastrophic event of MSD receiving numerous phone calls from upset members—who had received bills a year after the service had been provided. The average Monday call volume had risen from 1,800 to 2,600 calls per day. The average handling time for each phone call had risen as well—from an acceptable standard of 5.6 minutes to an unfavorable 7.2 minutes. The department continued to be kept inundated with these types of calls for the two years that these changes have been effect.

Although these impulses were atypical for Cahalan, she shrugged them off and attempted to continue her life as normal. Her “normal” life began to consist of incoherent rambling, emotional instability, and frequent seizures. Close friends and family decided it was time for a checkup, and convinced Cahalan to comply.
Tijanee M became self injurious, without warning she went to her room and placed a blanket over the door jamming the door for entry. Staff immediately attempted to open the door and was unable. Tijanee verbalized continuously she is going to kill herself tonight. The staff was able to gain entry to the Tijanee’s room with the use of the screwdriver. Tijanee was sitting on the floor with a sock tied her neck. Staff was able to provide emotional support and retrieve the sock from Tijanee’s neck. Tijanne began to pick at her recent sores until they were bleeding. Staff used kind gestures and hurdle help to calm Tijanee down. Tijanee was able to exit her room willingly and nursing was called for assessment.
Besides being showcased to the public as a medical rarity by her physician, patient L faced a multitude of other various problems with the Sutter Davis staff. When she first arrived to the hospital, the woman working at the front desk greeted her poorly. Patient L was told by the woman that “’[she] had to wait her turn,’ even though nobody else was there.” In the middle of filling out some medical paperwork, a young child and his mother walked into the hospital about ten minutes after patient L did. The Spirit Catches You and You Fall Down by Anne Fadiman is a nonfiction book that brings to light the clash of Hmong culture and Western medicine in Merced, California. Anne Fadiman tells the story of a Hmong immigrant family, the Lees, and the unfortunate condition of epilepsy that their daughter Lia suffers from. Throughout the book the reader sees great conflict inflicted on medical practitioners due to the Lee’s own cultural beliefs and the frustration suffered by the family due to miscommunication. Anne develops the story by giving a detailed background of the Hmong peoples’ lifestyle in their indigenous land of Lao, how it contributes to their beliefs, and their struggle to understand and accept Western practices.
Lia lee, a little Hmong girl, has a severe case of epilepsy. She is the daughter of Nao Kao and Foua lee who are among the many Hmong refugees that fled to the U.S. The Hmongs are very simple and proud people. They take pride in the fact that they have never been ruled by anyone. Even though they have been driven away and separated many times they always seem to find themselves and their culture, never seemingly adapting to the major culture. Even when French missionaries settled in their area and brought their western ideals, medicine, religion, and values they never really accepted them. Fadiman stated in her book that in reality, “no Hmong is ever fully converted” (35).
Within the U.S. Healthcare system there are different levels of healthcare; Long-Term Care also known as (LTC), Integrative Care, and Mental Health. While these services are contained within in the U.S. Healthcare system, they function on dissimilar levels.

In this case, communication and medications adoptions were the main difference in the treatment of a Lia. Though Lia’s parents and her doctors want best for her, but the above barriers were creating a hindrance in her treatment. They both were not understanding each other and interpreter was also not there, doctors want to transfer her to another best hospital because they were not getting with her disease but her parents misunderstood the situation and thought they were shifting her for their own benefit.
The Vietnam War caused great destruction in Laos, and so the Lee family migrated to America, after spending a short time in refugee camps in Thailand. After settling in America, Foua gives birth to Lia, who unbeknownst to them will suffer from epilepsy soon after she is born. For four years, little Lia is admitted to hospital seventeen times, after suffering both grand and petit mal seizures. Through miscommunication and a failure to understand each other’s cultural differences, both the parents of Lia, and her American doctors, are ultimately at fault for Lia’s tragic fate, when she is left in a vegetative state.
In Hmong’s, they have their own traditional beliefs in which they hardly ever alter due to a different atmosphere. Some of the Hmong beliefs are they prefer traditional medicine, are culturally active, host ritual ceremonies, and are spirituality influenced. In the book The Spirit Catches You and You Fall Down, refers to the Hmong culture and their beliefs on medicine while their baby Lia Lee, is suffering from epilepsy in which they have a hard decision. Traditional Hmong’s have their own medicinal beliefs which they obey prior to obtaining Western medicine. The gulf between Western medicine and Hmong health beliefs is an impossible abyss. Also, The Spirit Catches You and You Fall Down describes the life of Hmong refugees assimilating to the American culture which brings challenge to Hmong traditions.
A nursing home is another form of care and this involves an elderly person moving from their home to a building full of all amentities necessary for living well. The only downside to nursing homes is that they are the most exepnsive alternative and that there is a large waiting list for getting into one. In 2010, the Ontario Health Quality council reported, “wait times for a long-term care bed in Ontario have tripled since 2005” (Born, 2011). A nursing home has become a last resort because of these reasons and we need to do everything in our power to alleviate the amount of demand for these nursing homes.
